PT: I wouldn't recommend Embassy if physical therapy is required. Nursing: There's also a big powerful nurse that her coworkers are afraid of. She's angry at the patients that need nursing services provided, i.e., total lack of compassion, and no one in management has the courage to get her replaced. The good, conscientious nurses won't say anything critical about their cruel coworker, so the situation is endless. One star, because there's no such thing as zero stars. And one more star for a total of 2, for the good, compassionate, competent people at Embassy that do the best they can under the circumstances.

This star is for the very few employees at hearthside who actually give a crap 3 years ago my bariatric mother was there for rehab sent home without instruction or the equipment needed to care for her It was a grueling process getting everything so she could safely be in her home... Hospital bed wheel chair, lift chair... Nothing A few years later Here we are with a friend who was there for rehab after pneumonia that almost killed her 3 times Get the same old 3 day notice of release... When the day before they were still being told a couple weeks they'll be ready for release.... She gets released without the script for a nebulizer Which she needs for her breathing treatment I get it... Things aren't always quick, sometimes there's issues.. Ok But I call and call and call to be lied to blatantly After she's missed 3 treatments I get told we are working on it Well I'm not sure why breathing isn't at the top of the priority list Or why safety, mobility, care assessment and compassion have zero roles upon release The DON did not return my calls A nurse who actually needs 10 stars actually picked up the ball and faxed the order Which I completely appreciate and thank her There were numerous incidents where this client was treated horribly Tell her to use a filthy toilet with bm on it or they'll call social services??? Nice job!! I'm not placing all of the blame on this facility I know there's red tape insurance etc However, these "professionals" also have a duty Would not recommend this facility!!

Over the years, this facility has had its challenges, and it’s important to acknowledge that history honestly. That said, what stands out today is how intentionally and visibly leadership has worked to turn things around. The changes in care delivery, staffing, and accountability are not surface level, they are real and consistent. The focus on resident-centered care is evident, from improved clinical oversight to stronger communication with families and providers. Staff engagement has improved, and it shows in the day-to-day experience of residents. No facility is perfect, but this one has clearly committed to learning from the past and doing better, and it’s paying off. It’s encouraging to see a skilled nursing facility take ownership of where it’s been and put in the work to become a leader in quality care. That effort deserves recognition. Way to go to the team at Embassy of Hearthside!

The staff here have very caring hearts, some are new and others are long timers. Very family oriented. The facility needs some physical appearances updated, however Embassy the owner is making improvements.
