My mom has now been at Lodi Rehabilitation and Nursing for 9 months. Choosing a skilled nursing facility was not an easy choice, only because of my driving distance, but I knew this was about my mother and the care she is receiving. I continue to be impressed with the care, comfort and safety of the facility. ALL of the staff members are friendly, professional, attentive, kind, caring, and helpful. Communication is excellent and prompt by all. They also offer a variety of activities and services to help to keep my mom engaged and interested. I would highly recommend Divine Rehabilitation and Nursing at Lodi, in Lodi, WI. I appreciate all of the staff members and everything that they do.

We found the a perfect place to care for our father. The people who work at Divine are friendly, fun and take great care of our father. Even before the first day, my father was treated with respect, humor, sensitivity and empathy by top notch, well trained and directed staff. Any problem or concern we raised was handled promptly and effectively. The team at Divine goes the extra mile for their residents. I can't say enough good things about Ann Marie and the Divine staff especially the nutritionist who made the zucchini bisque soup.

The quality of the facility has declined considerably in the last few months. I am concerned that there was a $180,000 fine paid by the facility for a deficiency related to possible abuse. I know all nursing homes are short-staffed, but on a recent visit, door alarms were not functional and the back door was "broken", according to staff. I asked to have the room cleaned, as I could write my name in the dust, and floors were filthy and food strewn. I was told cleaning wasn't done regularly because my mom stays in her room and they don't want to wake her. The cleaning person was very nice and did clean some while I was there with the promise to do more this week. Calls are no longer returned, telephone mailboxes are full and when asked, no one seems to know who a director is. Apparently there was one who is no longer there and the current one is a temporary. I have tried to find ownership information, but all I can find is an Isaac Markovits, Ilan Richland and Wisconsin 3 Holdco, all without contact information. All appear to be east coast. I am not blaming staff. I know they mean well, but the turnover is high, the work is demanding and all I see is that this is a for profit company. It is time to put people ahead of profits.

Divine Rehabilitation and Nursing is an extremely clean facility, and provides compassionate care to its residents. Special activities keep residents engaged and happy. Nursing staff listens to what residents say, and there is quality communication between nurses and residents. We are grateful our loved one is taken well care of here.
