It was horrible and so dirty. Nothing to drink or a snack. No care was involved. Everyone was rude. The screamed at patience. I could not sleep because of the loud music and loud talking. Never again - never.

BEEARE!!! Please never let anyone you care about go to this facility. Case managers at hospitals should be made to go spend the night at Diversicare and endure the terrible smell down every hallway. The condition of the building is deplorable. The staff may be doing their best but I wonder why anyone would associate with such a place as this. My relative was finally moved to another facility after 2 weeks. He had bedsores that were never charted. He was not clean. He said they rarely did anything to help him. No bathrooms in rooms. Seriously, the state of Mississippi should do an internal audit and shut this place down. Awful place!

I see why there’s a big sign that says no photos or videos. They don’t want you documenting the neglect/abuse of their residents. It smelled strongly of urine and feces. There was a nurse who yanked a residents arm and slammed it on a table in front of me and did not care that I had just witnessed that. It makes me wonder what other atrocities take place there when nobody is looking. I would also like to mention we hit the call light when we arrived for a visit and nobody ever came the entire time we were there. The state needs to take a look at this place. I’ll be looking into hotlining.

This is not a facility I would recommend even to my worst enemy!! My FIL was a recent patient for short term rehab. The care overall was subpar. He/we had issues upon entry with the smells of the entire facility. The bathroom in his room was not clean and toilet overflowed on the first day. You could tell that the facility had not had any cosmetic maintenance in decades and when they did start working on it why in the heck did they do wall patch work and sanding while patients were in the room!! The meals were many times cold. The communication was siloed and inconsistent! The PT that he was supposed to have was incomplete and not just because he had restrictions upon arrival. Can you believe we were told that patient meetings counted towards PT hours! How in the heck is talking to the family for info sharing got anything to do with PT!! They were supposed to take him to a doctors appointment during his stay and even after being assured that all parties including transportation were in-the-know, it was not until 15 minutes before the appointment and after my constant inquiries of where is our ride did we find out the driver was not coming. This was an important visit with a cardio specialist that was required to determine next steps!!! The discharge process described to us at entry did NOT happen as we were told. It took almost 3 weeks to get equipment and home health support after discharge. If you have no other option but this facility Im going to pray for you and your family! Be sure to ask about the appeal process with Accenture. This was not shared with us clearly at all! He/we felt like things were not handled effectively or efficiently or satisfactory! AND after all this when we mention this is where he was for rehab to other we get the horrendous looks and stares of Really! Why! that place is not good!! Now if everyone is saying the same thing, what would you think?!

My father begged to go to the bathroom to move his bowels. Staff told him to "go" in his diaper! He's 81, w/c dependent, but deserves his dignity!!! The place is filthy and reeks of urine. My father's covered in an itchy rash (scabies?), and bruises. I have been a LTC and hospice nurse for over 25 years and this is one of the filthiest I have ever seen! He's on Medicaid.:-(
