This facility has the worst staff and nursing. They facility left a women’s elderly mother lying on the ground screaming for help after falling to the floor and did Nothing to assist or make her comfortable. If you love your family member you’ll think twice before sending here.

Currently my mother is in hospice care at this facility. The staff has been incredibly personable and has a care that is genuine to my family member. The process of getting my mother in the facility has been simplified by the clerical staff allowing me to be able to focus on all of the important aspects with minimal red tape issues that these situations seem to carry regularly. Thank you everyone that works here for all of your hard work and care.

I have visited other persons in rehab centers and know things are better than this place. No staff on the front desk. No one know where a patient’s room is. Staff doesn’t come when you need them. Left my friend after PT in a wheelchair for 3 hours before returning to get her in bed. I’m SO SO disappointed. I have a terminally ill husband and I wouldn’t put him here if he had to go to a place. I wouldn’t put my dog in this place.

I had a family member reside at this long-term care facility, and my experience was extremely concerning. During that time, I had repeated difficulty communicating with the administrator, Curtis, and the HR representative/social worker, Tammy. Phone calls, voicemails, and emails often went unanswered and were not returned. When visiting my loved one, I personally observed numerous residents sitting in soiled diapers for extended periods of time. In some instances, urine and feces had leaked onto the floor. I also witnessed insects on residents. On multiple occasions, I observed staff spraying chemicals in the room while residents were still sitting in soiled clothing, which appeared to be done to mask odors rather than address hygiene needs.In addition, I observed what I believed to be staff taking personal belongings from residents, including jewelry, money, and clothing. Based on my experience and what I personally witnessed, I strongly encourage families to thoroughly evaluate this facility and consider reporting concerns to the appropriate oversight agencies.

Diversa Care in Haysville has been such a blessing for my grandma and our whole family. From the very first day, the staff welcomed her like she was one of their own. They know her by name, remember the little things she likes, and always take the time to make her smile. The building is always clean and feels cozy, but it’s the people who make it truly special. The Director of Nursing is amazing—she’s not just in an office, she’s out there with the residents, checking in, lending a hand, and making sure every person is cared for like family. You can tell her heart is in this place, and it sets the tone for the whole team. They keep my grandma involved in activities she enjoys, she's so happy that she’s eating well again, and the staff are always patient and kind. Every time I visit, I can see how much she’s loved and cared for. It means the world to me to know she’s not just being “looked after,” but is genuinely happy here. She calls it her home, and I can see in her face that she means it. Thank you, Diversa Care, for giving my grandma such a safe, happy, and loving place to be.
