We can't say enough good things about this facility. They have really helped my mom, and keep her active. We are truck drivers she had gotten unable to care for herself. They have her up moving, and her labs have never looked better. We are glad We chose Diversicare.

Google should consider giving zero stars cause some facilities really deserve zero stars in this facility deserves zero stars I have experienced ongoing and serious issues with the level of care at this facility, from the Director of Nursing (DON) Aimee down to the CNAs. If you are considering placing a family member here, be aware that you must stay on top of their care at all times especially if your loved one cannot advocate for themselves. The DON presents a friendly, nonchalant demeanor, but concerns are not addressed meaningfully. Issues are acknowledged with a smile, yet no real follow-through occurs. The nurse manager, Amanda, appears pleasant, but interactions is none resolving problems. Another nurse, Jennifer, communicates in a manner to try intimidate as though attempting to scare or talk down to family members. The LPNs seem to believe their role is limited to passing medications and sitting at the desk. There is little to no supervision to ensure CNAs are doing their jobs. I personally witnessed CNAs get residents who are mobile dressed, place them in chairs, and then leave them there without proper care or follow-up. It happened to my family member When these concerns are brought to management, communication stops altogether. There is no accountability, transparency, or corrective action. Overall, the lack of responsiveness and oversight is extremely concerning. Please place your love one in another facility.

I'm only posting one star because It won't let you post 0 stars My father was admitted on Friday the 16th of May 2025 for rehab. When he arrived he could walk a very short distance and was coherent and knew what was going on. Saturday and Sunday my brother and uncle visited with him and he was ok other than being depressed. Monday I went to visit and he was tired and sleepy but not unusual for him. Tuesday nobody was there to visit but we did talk to him the phone and his speech wasn't great. Wednesday I walked in his room and I thought he was dead. He was laying in bed moving or making any noise. I get the aide in there and she checks him and has to clean him up due to laying in diarrhea for God knows how long. She brought to my attention that his urine in his catheter looked bad and said I think he has a UTI. NP comes in says same thing. Ordered lab work and said to would be next day before results are in. He had a bandage on his foot but nobody knew why. He had a bruise on his hip and nobody knew why. Today May 22nd his visitor for the day walked in and he was lying in diarrhea again had a fever of 102.7 was not coherent had dried caked in food between his legs his diaper and pants were messed up his catheter had been pulled out and was just shoved back in he had a bed sore on his butt his private area was raw like meat he had not been changed out of his clothes since Monday his urine was a milky color his face was red and sunk in his eyes were sunk in and dark underneath. I demanded for him to removed immediately and transported back home to Baptist hospital. Let's just say he's very very sick right now. How in the hell does this happen in a matter of 6 days? I saw a lot more going on that I didn't post in the review. I do have pictures of everything and I will be making a report ASAP. please for the love of God ... DO NOT send your family or friends here. This place needs shut down ASAP.

If I could do 0 stars or even negative stars I would! I have two loved ones in this facility and I am searching for a better place for both of them. This place is a joke. Call lights stay unanswered for 45 to an hour, sometimes completely ignored. The food they serve isn't fit for the residents, and barely meets the caloric intake needs of the people they claim to be taking care of here. This place has declined significantly over the last decade. Therapy used to be the best part about this place but even it has declined. They can't keep good workers. They punish good workers and rewards bad workers who bully and treat residents and coworkers like they run the place and they basically do run the place cause the leadership is laughable at best. I've got nearly 4 years experience with this place and I'm telling you, avoid it at all costs if you can and care about your loved one. Any good reviews you see are from current employees and those have been reported for conflict of interest. Please do not put someone you truly care about in this facility, it is a decision I regret and am actively trying to fix my mistake. Do not let them fool you with their fake smiles and empty promises that they have no intention of trying to keep. All they care about is the money they'll make from Medicaid and Medicare once your loved one is admitted. They won't even send a resident out to the hospital because they don't get paid if someone needs medical attention and has to be transported out to a hospital for help. Please do not even consider this place unless you hate the person and are just trying to dump them off somewhere to get rid of them. That's the only reason to ever put someone at Diversicare of Eupora.

My father was here because of insurance and at the time we were trying to work on getting guardianship. He was there like about a month. I think I fought with EVERY SINGLE EMPLOYEE about his care! I get he would do things in his room to harm himself and they had to watch him. But the day he passed I came to see him. The state he was in, in the day room was APPALLING! His eyes were completely covered in eye crusted, to the point he couldn’t open them! It was about 3 or 4 pm, they had him in a wheelchair leaning forward (he looked like he was going to tip out!!), he was unable to move or speak. I left, and an hour later they found him in the same wheelchair gone! (This was the weekend crew) I spoke to the week day manager and she told me they’d put him back in his bed after lunch because he’d get so tired from sitting up like that. I then asked do you not take notes on his care for the weekend employees? She said yes. So my father sat up til almost 6 pm or a bit after all day in a wheelchair!!? Someone that couldn’t lift his arms or feed himself! He didn’t pass away at least laying down? That is horrifying! I fought with those employees about things even after he passed. And the doctor never even spoke to me about him one time. Weird he couldn’t ever contact me. They are careless and the employees in the day room disturbing the patience with their gossiping is childish! I was told they would giggle loudly and one made a rude comment to my sister about looking like my dads wife not his daughter. You really need to work on empathy training and reevaluate leadership roles for part time positions. The person that cared for my father 5 days a week should have days off! Her notes should have been read and repeated by those employees. But they were just hanging out! Do better!
