This is a very nice facility. Being the owner of an unskilled home health agency I visit many SNF's, and I have always had good experiences here. I am especially impressed with Chanda in social services. Every time we have provided care to a discharging patient, she has done a phenomenal job keeping all parties informed and ensuring a pleasant transition. I would strongly recommend them to anyone in need of short term rehab or long term skilled care.

The nursing team was great. However, the rehab team did not meet my expectations. In 4 days of being there I only had 1 session of OT/PT the day before I was scheduled to leave.

I came to this facility over a month ago could barely lift my head off the pillow today i left using a walker, the therapist are excellent Lorraine Ariel Otavia and Kyra were unbelievable they are tough but always upbeat I felt like I would never walk again but they made sure I did, incredible team, the c.n.a.s also incredible sherry has the biggest heart in the world she norma, nicole and Julia treat you like family, they do a super tough job with little thanks but they are always smiling and willing to help you anyway they can they are very hard working woman, gabriel and Petria also always willing to help, fabuloso( im sure wrong spelling) and vernessa at night always helping and working hard, incredibly kind people here, yes it is old and run down the food is hit or miss and portions could be bigger but this is not a hotel and im sure there doing the best with what they have , chonda also is very kind and helpful, it's nothing fancy but if you want good therapy this is your place thank you all for your help.

I was truly shocked to see that the facility continues to provide its patients with such outdated televisions, and to experience the unpleasant odor throughout the building. These conditions raise serious concerns about the administration’s commitment to maintaining a clean, dignified, and caring environment for its residents. My family member was brought for physical therapy after surgery, its been a few days and he hasn't received any therapies.

Can't even give this place a star but had to give one to have review posted. Our family member in the beginning was treated "ok" in the beginning. Only one family meeting occurred during his stay. Case worker was very uninformed as to the guidelines of a nursing home and insurance guidelines. Told me she didn't have to talk to me when I had power of attorney and was listed as a contact for the patient. Patients were being discharged with no 30 day notification to the family, sent by Uber, or told they would be sent to motel since they had no place to go. Total disregard for safety of their patients. Not asking if there was someone at home to continue additional care at home or family meeting for this information. Administrator and Case worker tried to tell me all sorts of misinformation until I said I have worked in the medical field for 40 some years and know how all this works. Suddenly we got answers and extensions when needed. The food there is terrible and lacks proper nutrition....one slice of toast and a slice of bacon does not constitute breakfast!! They told us one thing and our family member told us an entirely different story. Nurse could not answer whether our family member was receiving a certain treatment, when they were actually receiving it 3x daily. Patient was discharged with no nebulizer, the walker that was order was incorrect. Instead of calling prescriptions in for a patient who does not drive, they issued paper prescriptions. Nothing was done to make the transition home comfortable for the patient until I threaten to call my attorney. Horrible horrible place!!! Administration and caseworker should be terminated. The state should definitely investigate this place. They only good thing I can say is most of the nursing staff and PT was good to my family member.
