I have read a couple of the negative reviews and I just wanted to share a very positive experience my family had at Crestwood nursing facility from mid January to mid Feb 2024.,I can’t say enough good things about the care my dad received at Crestwood. He was sent initially from RI Hospital to another skilled rehab facility for 10 days before I transferred him to Warren Rhode Island. My family and I loved the atmosphere of a home environment and all of the staff members on the upper level at Crestwood were very kind and considerate to my dad and my family. As an out of state rehab RN myself, I was very impressed with the compassionate care by both the nursing and the therapy staff. He received excellent PT, OT, and speech from a group of therapists, who truly cared about my dad’s ‘s progress, despite his limitations from his advanced dementia and Parkinson’s. Dad’s disease progressed & he developed serious complications. Sadly. we had to transition him from skilled care to hospice. The nursing home staff showed so much love to my dad. Both they, and the hospice staff who came in daily, made my dad’s last 8 days so comfortable and peaceful. We also received emotional support from the administrative and social work staff which was greatly appreciated. Thank you Crestwood!

I placed a loved one at Crestwood Nursing Home a few years ago, and six weeks later my loved one passed away from the hands of gross incompetence at this facility. Once arriving there, they were placed in one of the basement rooms with an unsuitable roommate. I asked for a move upstairs, but that never happened. Their diet got modified, even though their chart indicated that a diet of mashed or ground foods needed to be followed, due to Advanced Dsyphasia. Instead, I would see my loved one being served things such as hamburgers, spaghetti, and pizza. Saw them the night before, and my loved one was up walking, talking and laughing, only to get a call the next day, saying they couldn't get out of bed couldn't walk, and they would be tested for a UTI, as that what it probably was. Started a regimen of Antibiotics. When the results came back negative, they continued them on the antibiotics, and had the AC running constantly close to my loved ones bed. Was told by the daytime CNA that my loved one was fed pancakes and apple juice for breakfast, even though my loved one slept throughout the entire weekend. When I saw no improvement the following day in my loved ones condition, I said to the RN on duty I wanted my lived one going to the hospital. The nurse simply said I'm,I called their resident Dr., then called the rescue. When we arrived at the hospital, the attending Dr., said that my loved one had hours to live, and was aspirating(choking), and was battling Sepsis, and kidneys were shutting down. The hospital staff worked tirelessly to save my loved one, but too little, too late, and they died two days later from Aspirational Pneumonia, Sepsis, and Renal(Kidney) failure. Crestwood grossly misdiagnosed their condition, and failed to provide the appropriate treatment. Sadly, moving my loved one into this facility, where I entrusted them with the life of my loved one was a huge mistake. Stay Away from this place!!

This is not a place to stay after a hospital for rehabilitation. No one helps you try to get on your feet to walk and get better. The therapist at work would you have no experience what's ther in therapy. You spend most of your time in bed and that's where they leave you.
