We were responsible for my husband's uncle's care. He was abused at an assisted living facility in Green Bay. We moved him to Countryside so we were close to manage his care and ensure he was not abused. Every assisted living and skilled nursing facility struggles with staffing. It's a tough business. As a nurse, I was particular about a home for my husband's uncle. What we experienced with the abusive facility was awful. Any place we selected would be carefully scrutinized.

Rooms at Countryside are spacious and have windows so there is lots of light. Housekeeping does a great job of keeping the entire facility clean. Overall decor is lovely. We had a huge issue at the abusive facility with laundry being lost. Laundry staff at Countryside treat residents' clothing with respect and nothing was lost, not even a sock. Laundry let us know if something was stained or needed repair. Maintenance was responsive and even came in on a weekend when something needed repair. We were impressed with the dedication and attention to detail.

My husband's uncle was particular about food. He liked the food at Countryside. Right before he started hospice care, he stopped eating. We were so impressed when the cook came to his room, concerned he wasn't eating and offered to make him whatever he felt he could eat.

I cannot say enough nice things about Countryside's staff. Caregivers and med techs obviously care about the residents. If a family member resides in assisted living, take the time to get to know the caregivers and med techs. They are your eyes and ears to how your loved one is doing. Say thank you - a lot - -because their job is difficult and the industry is not known for its high pay. They are doing this because they truly care about the elderly. Caregivers knew our loved one so well that when he was dying, they kept switching the television channel to Fox News, his favorite, from the classical music we left playing. We asked a caregiver to please tell staff not to change the channel. The fact caregivers knew his favorite television channel tells you how much staff care about residents. I stayed at the facility from morning until 4a the last few days of his life so he wouldn't be alone. Staff regularly checked to see if I needed anything and helped reposition and change him. It was a difficult time and their kindness was greatly appreciated.

Becky, the front desk person; Alex in admissions and Laurie in activities plus the dietary staff were wonderful. While our family member was there, administration changed. While this can be a red flag, we can honestly say our loved one's care remained excellent. The two administration teams were dedicated and professional.

My husband's uncle passed away in February 2024. We miss him but are very grateful we found Countryside for his last years on Earth.

We are very disappointed in Countryside Manor & don't believe this is a true memory care facility.

They did not have the staff or education to keep our loved one safe without requiring us to hire additional 24-hour care. The administration threatened us with eviction if we didn't immediately comply. We were not aware that anything our LO was doing was anything out of their experience or expertise until they called me at work on a Friday afternoon during a holiday weekend, making threats.
We complied since we were left without other options but started searching for new facilities. That's when I learned someone from the administrative team told a prospective facility that we as a family refused to medicate our LO. This is a lie. Now we are wondering if this was said to make our LOs acceptance into a new facility difficult.

The turnover rate is high, with at least 4 administrative teams in less than 2 years.

It wouldn't be fair to say it was all bad. Many staff members are phenomenal, from the activity director to the caregivers to the people at the front desk. Amazing. At one time, we gave this facility a five-star rating, but after how we were treated, we can no longer. I hope the administrative team learns from this mistake and is transparent about what they can and can not accommodate.
