The level of incompetence was staggering. They failed getting my spouse the appropriate prescribed medications, most notably Warfarin for 12 days, as per the St Charles COAG clinic in Redmond. The "registered" nurse suggested that my spouse needed to go to a chiropractor for his knees, when in fact he had a fractured pelvis, as per the St Charles Redmond ER. They missed a black toe which led to sepsis (time-stamped pictures available). They stole his clothes out of his closet and dumped them in a bin in their MedTech room so that he did not have access. We had another incident that was illegal as per Oregon law. ETC.!

There are few things in life harder than making the decision to place someone you love more than life in the hands of others. There are not enough words to express our gratitude to the entire staff at Countryside. The kindness and compassion displayed by all of you was so genuine and heartfelt.<br>On behalf of my siblings, my father, and our entire family, we thank each and every one of you for treating our sweet mom with the utmost dignity and respect.❤❤
