Absolutely a horrible place. Listed as BELOW AVERAGE (WORST) rating through medicare.gov as well as Health US News. Currently opening an investigation. They paid $45,713.00 in fines in the last 3 (THREE) YEARS. Staff turn over 57.1 percent since previous year, only received a 2 on Fire safety and emergency procedures. I could go on...... investigate before placing anyone in this place!

Great four star facility with a great therapy team. Facility accepts MCR/MCD and most insurances. They do long term and short term care. All private rooms.
3 inspection visits in the last three years, 35 citations on file. Inspections are conducted by state agencies on behalf of the U.S. Centers for Medicare & Medicaid Services (CMS).
Have a registered nurse on duty 8 hours a day; and select a registered nurse to be the director of nurses on a full time basis.
