We had an outstanding experience with the care provided at Chatham Hills Subacute Care Center when our family member suffered a fall and required an extended stay. The facility was clean, well-maintained, and it was evident that the staff genuinely prioritize creating a supportive and healing environment. Everyone we encountered was consistently kind, attentive, and professional. They take the time to listen, make patients comfortable, and treat each person with the utmost respect. The team demonstrates an unwavering commitment to patient safety. James, the administrator, places patient well-being above all else and takes an active interest in patients as individuals. Alex, the Director of Admissions, is deeply passionate about his role and consistently goes the extra mile to make certain that every need is met and every concern is addressed, whether or not it falls within his direct responsibility. His open-door policy makes families feel truly welcome and heard. Director of Nursing Janelle brings years of experience and confidence to her leadership, and night nursing supervisor Erick is prompt, polite, and extremely knowledgeable. Dr. Chakilam and Dr. Bachar are approachable and are invested in delivering high-quality care. There is a saying that food is medicine, and Michael, the Director of Culinary Services, and Sun, the clinical dietitian, exemplify that philosophy. They put exceptional effort into making sure meals are nutritious, balanced, and enjoyable. In a care facility, good meals can be a key motivating factor in physical rehabilitation, and Michael and Sun understand this deeply. Michael personally shops for high-quality ingredients that even a food connoisseur would appreciate, going above and beyond to make meals appealing and thoughtfully prepared. After meals are customized, Sun regularly follows up to learn which foods were preferred and finished, and what adjustments can be made to meet nutritional needs. The patient care aides treat residents with the same compassion and attention they would give their own family members (special mention to Adwoa and Evelyn, though there are many others). They are also wonderfully warm with visiting children, welcoming them as if they were their own grandchildren. The rehabilitation staff are encouraging and supportive, helping patients remain motivated and engaged throughout their recovery. Michael, who arranges patient transportation for physician appointments, is thorough and takes care that every detail is carefully coordinated for a safe and comfortable ride. There are many additional staff members we hold in very high regard, and we are deeply grateful for the dedication and professionalism of the entire Chatham Hills team. We credit the facility with our relative’s remarkable recovery. At 86 years old, he was able to regain meaningful independence and return home to us. As a family of physicians, we carefully chose Chatham Hills, and we give this facility our highest recommendation to anyone in need of subacute care.
