We had an outstanding experience with the care provided at Chatham Hills Subacute Care Center when our family member suffered a fall and required an extended stay. The facility was clean, well-maintained, and it was evident that the staff genuinely prioritize creating a supportive and healing environment. Everyone we encountered was consistently kind, attentive, and professional. They take the time to listen, make patients comfortable, and treat each person with the utmost respect. The team demonstrates an unwavering commitment to patient safety.
James, the administrator, places patient well-being above all else and takes an active interest in patients as individuals. Alex, the Director of Admissions, is deeply passionate about his role and consistently goes the extra mile to make certain that every need is met and every concern is addressed, whether or not it falls within his direct responsibility. His open-door policy makes families feel truly welcome and heard. Director of Nursing Janelle brings years of experience and confidence to her leadership, and night nursing supervisor Erick is prompt, polite, and extremely knowledgeable. Dr. Chakilam and Dr. Bachar are approachable and are invested in delivering high-quality care.
There is a saying that food is medicine, and Michael, the Director of Culinary Services, and Sun, the clinical dietitian, exemplify that philosophy. They put exceptional effort into making sure meals are nutritious, balanced, and enjoyable. In a care facility, good meals can be a key motivating factor in physical rehabilitation, and Michael and Sun understand this deeply. Michael personally shops for high-quality ingredients that even a food connoisseur would appreciate, going above and beyond to make meals appealing and thoughtfully prepared. After meals are customized, Sun regularly follows up to learn which foods were preferred and finished, and what adjustments can be made to meet nutritional needs.
The patient care aides treat residents with the same compassion and attention they would give their own family members (special mention to Adwoa and Evelyn, though there are many others). They are also wonderfully warm with visiting children, welcoming them as if they were their own grandchildren. The rehabilitation staff are encouraging and supportive, helping patients remain motivated and engaged throughout their recovery. Michael, who arranges patient transportation for physician appointments, is thorough and takes care that every detail is carefully coordinated for a safe and comfortable ride.
There are many additional staff members we hold in very high regard, and we are deeply grateful for the dedication and professionalism of the entire Chatham Hills team. We credit the facility with our relative’s remarkable recovery. At 86 years old, he was able to regain meaningful independence and return home to us. As a family of physicians, we carefully chose Chatham Hills, and we give this facility our highest recommendation to anyone in need of subacute care.
So very grateful my brother went to Chatham Hills for Rehab. The “gold nugget” here is the caring staff ( doctors, physical therapist, occupational therapists, social workers, & nurses). Yes, the facility could use an update but that is NOT what is important. The PT team (Victor, Steven &Connor) inspired him to work hard and get back on his feet again. Dr. Prager & assistant AJ provided great medical oversight. The social workers, Cynthia & Saba made an easy transition at discharge. A big thank you to the director Alex and the many day and night nurses that made his amazing recovery possible.
I am doing this review so that I can give Michael Curtis praise for his hard work in creating food that my elderly mother enjoys on a daily basis. It’s a hard stage of life and his effort and consistency as well as accommodating nature makes it easier on both myself and my mom. It’s a comfort knowing she can enjoy flavorful and nutritious food. I want to take a moment to share my experience Chatham Hills Subacute Care Center where my grandmother resides. The overall staff and atmosphere was delightful. I had lunch with her and the dining and kitchen service experience is truly commendable. I believe the staff deserves recognition for their hard work.
From the moment we arrived, I was impressed by the welcoming atmosphere created by the kitchen team. Mike, Marcus, and Nick, consistently prepares meals that not only taste wonderful but are also presented beautifully. My grandmother often talks about how every meal is a delightful experience, with a variety of options that cater to different dietary needs.
Overall, the combination of delicious food, a caring kitchen staff, and attentive service has made a positive impact on my grandmother's quality of life here. I highly recommend Chatham Hills Subacute Care Center for anyone looking for a place where the residents are treated with kindness and respect, especially in the dining room!
I’m here as a Chatham Hills Employee and also a patient’s family. Just before the Thanksgiving holiday my mother in law who I love love love took a tumble at home which caused her blood pressure to drop, she also had dizziness, confusion and weakness. During her hospitalization she was recommended to go to rehab for her current issues but more importantly for an untreated condition of lymphedema in her legs which was the cause of her fall (legs so large she was not able to move around her apartment and limiting her mobility she’s still undergoing her therapy but as a family member I’m amazed at how my team has handled her condition from day one. Jannell our DON, Christine, Unit manager, our Rehab team, our Medical Director Dr Prager, our Cardiologist Dr Saeed, our CNA’s who are all so attentive, our Podiatry Dr Escandar who speaks to her in her Arabic language, immediately made their recommendations, setting her program and goals, she’s now on day 15 her legs have reduced 3 sizes and still improving she now walks up to 150 feet with her rolling walker, smiles and enjoys the activities, and social interactions she’s been getting she enjoys the food which our dining team tailors to her taste and adds a little flare for her culture above all I knew the team was amazingly put together by our Administrator James but watching them in action from the family side line has been such a site to witness and my husband and I are so grateful we are looking forward to her being home for Christmas #TeamChathamHills #RealResults
