It's been a Rollercoaster since july 1st. This place was great my ex was only their for 5 days but the staff is exceptional compared to the 2 other places hes been, and they finally got him transferred to Fresno, which is what we waned from day 1, thanks to ranjib, you guys are Great. We almost don't want to transfer him since they are great, but this way his family can see him more, Thank you all again

Please I beg, if a patient needs long term placement DO NOT send them to Ceres Post Acute. It is an awful place. I feel compelled to share our experience in the hope that it prevents another family from going through what we did. My father was transferred to this facility for post-acute care. Within a very short period of time, there was a noticeable and alarming decline in his condition compared to his status at the prior hospital. He went from being able to communicate, eat, and walk with assistance to suddenly struggling to breathe and unable to speak in full sentences. When concerns were raised with staff, communication was extremely difficult. We were unable to get clear or consistent information about his condition, medications, or care plan. Requests to speak with a physician were delayed, and when a physician did respond, we were told everything was “fine,” despite obvious signs of decline. There were unanswered questions about medications being administered that did not align with what we were told his medical conditions were. I was requested to call the previous hospital and determine why. This felt very inappropriate to inquiry rather than being a task handled by the facility. On the day of his passing, we were informed earlier that his vital signs were stable and that he was responsive. Within a very short window of time, he was found unresponsive and resuscitation efforts were unsuccessful. The facility could not explain how or why his condition deteriorated so rapidly, nor could they provide a clear cause of death. We were also given incorrect and confusing information regarding post-death procedures, which added unnecessary distress during an already devastating time. Afterward, my brother went to retrieve his belongings and described the facility as unsanitary and chaotic. There was a strong odor of urine, constant unanswered call bells, patients visibly distressed and unattended, and significant delays just to gain entry to the building. Communication with staff afterward was also extremely poor, including being disconnected during phone calls when attempting to seek information. The lack of accountability, poor communication, and overall condition of the facility were deeply troubling. If you are a family member advocating for a loved one who requires long-term or post-acute care, I strongly urge you to thoroughly research and consider other options. I would never knowingly allow a loved one to be placed here again. My heart goes out to other families who may find themselves in similar situations. My father was only in their care for a total of 5 days.

I can't express enough gratitude for the exceptional care and support my uncle received during his rehab stay at Ceres Post Acute following his stroke. From the moment he arrived, the entire team made the process smooth, comforting, and, most importantly, effective in his recovery. The nursing staff was absolutely outstanding. Every nurse showed remarkable compassion, patience, and dedication to my uncle's well-being. They were always attentive, addressing any concerns promptly and ensuring he was comfortable every step of the way. Their professionalism and kindness made a difficult time much easier for our family. A special thank you to the Director of Nursing Kiran, who went above and beyond in providing personalized care. Not only did she lead her team with exceptional skill, but she also made sure to stay in close communication with our family, offering updates and guidance when we had questions. Her leadership was truly inspiring and gave us peace of mind. The therapy department was incredible in helping my uncle make significant progress. The therapists were not only highly skilled but also incredibly motivating, making the rehab process both productive and encouraging. They were always positive, pushing him to reach his goals, while making sure he felt supported and capable throughout. We were also thoroughly impressed by the management team. Their coordination made everything run seamlessly, from admissions to discharge. The team was highly organized, and we could always count on them for quick responses and detailed answers to our questions. The admissions team, especially CJ was so welcoming and professional during the intake process. They made sure that all paperwork was handled efficiently, ensuring there were no delays in getting my uncle the care he needed. The intake team’s friendliness and thoroughness really stood out, making the transition into rehab smooth and stress-free. Lastly, Jennifer in the billing department deserves a mention for being transparent, efficient, and supportive. They worked directly with us to clarify any financial concerns, and we always felt confident in the handling of the billing process. Overall, the care my uncle received at Ceres Post Acute exceeded all expectations. This facility is truly a gem, with a dedicated team of professionals in every department. Their unwavering commitment to quality care and the well-being of their patients has made a significant difference in my uncle's recovery, and for that, we are endlessly thankful.
