About Liberty Village of Centralia

Liberty Village of Centralia is a 120-bed skilled nursing and long-term care facility in Centralia, Illinois. The community serves residents requiring rehabilitative therapy and ongoing nursing care.

Family experiences at the facility are deeply divided. Positive reviewers consistently praise individual staff members, particularly nursing and therapy staff, for attentiveness and compassion, and note that the facility is clean with good food service. However, numerous families report serious concerns: poor rehabilitation outcomes, inadequate monitoring of resident status and medication administration, lack of communication from administration, and maintenance and housekeeping issues including unresolved facility problems left unaddressed for days. One family noted that "we will have to get back to you" was the response when calling to check on a mother's medical status. Operational issues extend beyond patient care, the facility reportedly failed to pay an external service provider for months despite repeated calls.

The community holds a 3.4 out of 5 Google rating based on 16 reviews, with ratings clustering at both extremes: five 5-star reviews highlight dedicated staff and cleanliness, while six 1-star reviews cite communication failures, care quality concerns, and operational dysfunction. The stark contrast in family accounts suggests inconsistent experiences across departments and shifts.

CM is very fortunate to have Roberta S. LPN. While my aunt was in hospice Riberta was kind, compassionate and went out of her way to preserve my aunt’s dignity. Roberta is nursing excellence!

What a great place! The staff has been amazing, food is good, the CNA’s are precious watching them care for people and Jennifer, the Administrator, has been nothing but gracious and professional. I would recommend this place to anyone.

I did a service for this facility back in September, have not been paid and when I call administrator have been told numerous times that the check is in the mail. That has been 4 months ago. They will not tell me where they sent it, when it was sent, did it go to wrong address and someone else cash the check??? The other staff at this facility was and is very nice and friendly, but for whatever reason, the administrator keeps putting me off.
