About CenterWell Home Health

CenterWell Home Health provides home health and hospice services in Buffalo Grove, Illinois. The organization delivers skilled nursing, physical therapy, occupational therapy, speech-language pathology, and home health aide services to patients recovering from hospitalization or managing chronic conditions at home.

Families consistently praise the physical therapy staff for their professionalism and dedication to patient progress, with one describing how the physical therapist "helped me walk better with my walker and helped me get stronger." The nursing team receives particular recognition for attentiveness during wound care and post-hospital recovery, as well as for proactive communication with physicians. Therapists are noted for calling ahead to confirm appointments and treating patients with genuine warmth and respect. One family wrote that staff "treated my mother like she was family."

However, some families report inconsistent experiences with occupational therapy and speech-language pathology services, including incomplete visits and delayed or missing appointments despite active orders. Home nursing assessments have occasionally fallen short of expectations, and scheduling reliability varies across the care team.

CenterWell Home Health holds a 4 out of 5 Google rating based on 20 reviews, reflecting strong satisfaction with physical therapy and nursing services alongside notable gaps in coordination and follow-through for specialty therapies.

I want to thank the nursing team that visited me after my hospital stay. My physical therapy nurse and regular health nurse were really nice to me. They answered questions that I had and took their time in helping me. Again, thank you for the medical service that was provided to me.

We had expert care by the occupational therapists and great care by physical therapists. Centerwell was available and helpful when we called and had questions. We would recommend them.

The Nurse & Physical Therapist have been wonderful. The nurse has been an interracial part of my health by calling Doctors if there is something wrong. The physical therapist has helped me walk better with my walker and has helped me get stronger! Thank you CenterWell Home Health

PT, OT, SLP, home nurse, and aide were ordered. PT very good. Swauvic excellent. Some sessions were a bit short time wise but he is very good and professional. OT not helpful at all and only came 3 times then stopped despite orders. SLP never came and after 1 month I called to ask if they were coming. Home RN came several times for wound care but did not assess patient fully, then despite open wound signed off. 2 weeks later wound bleeding excessively I had to call centerwell urgently and they sent a RN who didn’t seem to know how to apply a pressure dressing and did not have any of the needed supplies. Aide never came. Initially Centerwell seemed good but lack of services became apparent.

Yes.
