This is an amazing facility! The nursing staff are wonderful and provided excellent care for my mother-in-law after she was hospitalized. We are very grateful for the care and support she received and would highly recommend this facility.

My mom was here for a few months and they took horrible care of her. The staff were mad rude. They also got rid of a blanket we had made for her. I would not send your love ones here

Changing the name From Heberts will not change the fact this place is a disgrace. As long as Linda is in charge I would never put a loved one in this establishment

Sadly, I had to select even 1 star.<br><br>My mother has been left in a room and ignored. She has urinated on herself after multiple hours of waiting for help to the restroom. Her physical therapist arrived while she was still asleep and left, never returning for the day!<br><br>I have attempted to call the facility 14 separate times in the last 2 days and not a single call was answered.<br><br>Keep looking! Keep trying for a different facility! Don’t leave anyone you care about, or don’t for that matter, at this facility!

Thanks to the Team at Lake Forest, my mother was able to return home stronger and more independent. I highly recommend this facility to anyone looking for quality rehab and genuine care for their loved ones.
