Carriage Square is a beautiful facility with amazing staff who truly care for their residents. This is a great facility for anyone looking to make it a home away from home for their family/loved ones.

Therapy department is top notch! They encourage, they are positive and the residents enjoy working with them. When you have patients that need to return and make sure out therapy dept want changed, that speaks volumes! Our Activity Director makes sure to keep the calendar full for everyone to ensure all needs and wants are being met! Just 2 reasons to come to carriage!

This is a terrible place. Do not send your loved ones here. They let my grandmother fall twice on a 24 hour period. She had stitches above her eye, concussion, broken collar bone, broken elbow, broken hip, uti, and pneumonia. I was deviated. I would NEVER recommend this place to anyone. They can’t take accountability and blamed my gma for not asking for help. My grandmother was completely able to walk on her own before she got there now she can’t do anything without help. They destroyed her quality of life.

When my husband entered Carriage Square in December 2024, the care was not bad. As time passed, things got progressively worse. The food was almost inedible, the follow through from most nursing staff was poor, and they were not following the nursing orders, such as getting him out of bed daily. The excuse was usually that they "did not have the staff." During the month of March, he was given only 1 shower a week. There were 3 different administrators during his time there and I spoke with 2 of them regarding his care with little response. I filed an official grievance and received no follow up. The ombudsman was contacted multiple times with limited, temporary changes. There were some exceptional staff, including Colton, Bailey, and Ricki. The physical therapist, Rob, was also wonderful. These 3 provided excellent care despite roadblocks from the actual facility. I would not recommend Carriage Square unless major changes are implemented.
