I want to thank everybody. To the Nurses and CNAs. I would also like to thank Adam for training me to get better, my CNA Isabel, Raj in Therapy for giving us good information about my situation and Machu for the fun Bingo. I also want to thank the case manager Mayra for her help, she was so kind to me and helpful. As an 80 year old patient, I encouraged her to keep studying to be able help more people like me. In the beginning, I did not understand the system to be in Nursing home. Now I understand everything and I’m going home happy.

I would like to take the time to praise 3 exceptional Canyon Creek employees, Peachy, Montana, and Margot.<br><br>I would like to praise Peachy for her outstanding work and empathy as a social worker and human being. She is warm, caring and knowledgeable. She does her homework and responds immediately to phone messages, texts and emails. Peachy helped us navigate the ins and outs of Canyon Creek's Post Acute care. She stopped by daily to check in on us. When she was unable to find a place for mom close to my home, she referred us to another social worker in my area, but she didn't desert us. She has continued to work on mom's behalf and mine. I am truly grateful that Peachy has come into our lives.<br><br>Montana has gone out of her way to treat mom's ongoing itchy rash. Mom has had those rashes for over a year. Many have tried to make it go away and have failed. Mom's medical needs do not allow Montana or others to treat the rash with just anything, but that didn't stop Montana. She took the extra time to do research and to consult with a doctor. She monitors and nurses mom on a daily basis applying ointments where needed to mom's delicate skin. With the doctor"s consent she made-up a special topical cream for mom. Although mom's rash is still present, it is almost gone. Nobody has done a better job in giving mom relief than Montana.<br><br>Margot is mom's amazing occupational therapist. Mom couldn't even sit up when she first arrived at Canyon Creek, but Canyon Creek has an amazing staff of therapists. It began with Margot's patient and caring manner that slowly coaxed mom to first sit up and to eventually walk with Ralph"s assistance. Mom is actually enjoying physical therapy. Margot not only cares for mom's therapy needs but she cares about all of mom's needs. She communicates well with the two of us and with other medical professionals about her concerns and observations regarding mom. We value her warm heart and vast knowledge, and appreciate everything she has done for us.<br><br>These three women, Peachy, Montana, and Margot are shining examples of medical professionals that "work with heart, lead with purpose, and serve with impact."

I came in Canyon Creek with major trauma to both my legs. I was very confused and frustrated. It took me awhile to get used to the facility but overall the staff I would give an overall rating of an eight some of the staff members that went above and beyond Nadine Josephine Arlene Ralph Missy and Kevin a very young boy that really loves what he's doing CNA. I really feel our big hospitals like Kaiser are failing these facilities and not giving them enough support I would give Kaiser a two out of 10. I will go home today and I hope I will add another review after I look at more of my notes
