Brookdale HealthPlus Helps Improve Your Care Experience

HealthPlus is our exclusive healthcare service that allows us to address your needs with more personalized care. When you need medical care, our Care Manager, a registered nurse, will coordinate the services you need with your healthcare providers of choice. The Care Manager and care associates will share information with your doctor(s), so everyone on your care team will be on the same page making life easier for you.

We have a relationship with doctors and nurse practitioners who can provide services to you on-site, so you have access to routine and comprehensive care right here in the comfort of your community and may avoid trips to the doctor's offices or unnecessary emergency room visits and hospital stays.

That means you’ll have access to:

    Convenient, preventative care services, like annual wellness visits, immunizations, and health screenings
    Support for chronic health conditions, such as diabetes, high blood pressure, congestive heart failure, or COPD
    Help to manage urgent care needs with oversight of clinical triage and urgent care protocols
