If you love your family please don’t send them here. Prepare for your loved ones to be left soiled for hours because the facility is understaffed. Feeding is rushed so if your loved one needs extra time with food assistance, they will sometimes go without. The facility is extremely dirty (roaches and rodents) and a blatant disregard for staff washing hands. There was a mouse squealing in a glue trap out in the open for everyone to see in the hallway and I was yelled at by staff for bringing attention to it. Don’t get me wrong there are great nurses and CNAs here, I think management and staffing are to blame for the most part.

As my father was there for over 4 weeks, I cant say enough about the care, the compassion they displayed, my mom and I would like to thank ,Alexis,Aaron,Ericka,Kamiaha,Zoey, and Cynthia, and Deidra, We can't Forget William and Cheryl and many more that I can't forget that affected my father's life, they were above and beyond what anyone could expect !! The Rehabilitation Dept was awesome also, First class operation, Thank you to All!!!!!!

If you value patient rights, dignity, or basic human decency, avoid this facility. My mother was admitted here for stroke rehabilitation, and despite being fully mentally competent, mobile, and medically stable, she was subjected to manipulation and intimidation when she tried to leave. In 11 days, she received a grand total of five hours of physical therapy — and yet the facility pushed to keep her longer simply because her insurance approved more days. When she expressed that she was more comfortable at home and wanted to be discharged, staff became hostile and coercive. Most disturbing of all, they threatened my brother with “arrest” for simply coming to pick her up — a completely baseless, unethical, and inappropriate attempt to scare our family into compliance. A rehab facility has no legal authority to detain a competent adult or to threaten family members who support a patient’s decision. Their behavior was not only unprofessional, but dangerously close to patient rights violations and false imprisonment. The fact that they used fear tactics instead of quality care or honest communication tells you everything you need to know. I am on my way to pick up my mother, and I dare them to threaten me with the police. My mother will leave safely, willingly, and competently — despite the facility’s efforts to intimidate her. The care was minimal, the communication was poor, and the staff’s behavior was unacceptable. Families deserve transparency, respect, and real rehabilitation — not manipulation. I would not recommend this facility to anyone.

My mother was sent here for palliative hospice care during emergency respite. We pulled her out in less than 24 hours. Mom was left UNATTENDED from her arrival, between 7 - 8 PM, until our hospice sent on-call nurse arrived for a welfare check, at 11:30 PM. Assessment included elevated blood pressure, anxiety and respiratory distress. She had been given NONE of the medication in her chart, now required within 2 hours to prevent further suffering. The rehab claimed that her meds were supposed to have come with her and that no order would be filled (as I have been told by multiple sources present on my mother's behalf). Since I am out of state, I asked a trusted friend to bring Mom her medication. The staff would not let her leave it due to being in the daily sorters they had been distributed into prior to the emergency respite order. Our friend assisted her with that evening's doses and left with the medication she brought. Inquiries made of the staff on duty about orders for my mothers medications were met with conflicting and unclear information. One nurse had Mom's FULL med list and orders and indicated that they were being filled, while the nurse "behind her" in the station stated "Absolutely not." I DO understand and respect why presorted medications were not permitted to be left. Instead, we should have been able to obtain determination IF or WHEN Beachside would fill her order. There was 100% INABILITY TO REACH ANY STAFF MEMBER INSIDE OR OUTSIDE OF BUSINESS HOURS. From the time of her transport from home, even hospice staff was unable to directly contact ANY staff member. When the ONLY PUBLISHED phone was answered and I requested to speak with the nurse tending to my mother, I was transferred into a corporate voice mail retrieval service. I contacted our hospice provider and requested another visit to my mother in this facility. Mom's hospice nurse arrived and called me at 12:30 PM, sharing in my distress and concern over my mother's condition and lack of care. My cachexic mother (EXTREMELY MALNOURISHED, AROUND 80 POUNDS, symptomatic of her current condition) had NOT been offered ANYTHING MORE THAN WATER AND MORNING MEDS. NO BREAKFAST or FOOD. She was in respiratory distress with only her O2 and a rescue inhaler that was left with her overnight. Her blood pressure was elevated and anxiety was disturbingly high. MY MOTHER expressed to me that she was left to wait for assistance to the toilet FOR OVER AN HOUR. (She is a fall risk and being treated for a UTI - ALREADY IN POOR AND DECLINING HEALTH.) Still, Beachside Rehab was unresponsive and unclear in the minimal response (only to the hospice nurse - NEVER TO ME) to providing clear explanation of what THEY would or would not provide, in spite of the ORDERS FROM HOSPICE. Urgent need messages were sent to our hospice case manager expressing our concerns and requesting additional direction. This nurse had to leave Mom until another of our hospice nurses would be back in the afternoon for follow up. The next nurse contacted me immediately. She requested my personal knowledge and experience since this had begun. I learned that mine, Mom's and our (multiple) hospice nurses' accounts were in alignment and matched the current nurse's observations while the rehab account appeared nearly completely contradictory to the real time observations. We were offered 2 options to choose from: 1. Hospice nurses 3 times daily for monitoring, confirm meds administered, care needs met while staying at Beachside; or 2. Bring Mom home by non-emergency medical transport. Needless to say, we had already chosen option 2 even before it was fully vocalized. The impact of this experience is painful and it is yet to be determined if it is recoverable to her condition 48 hours prior to this review. My recommendation is to stay far away. FAR FAR AWAY

Beachside is the BEST of the best! Their facility, staff, and most especially admissions, are top notch! They’ve helped us every step of the way! Everyone is incredibly warm, compassionate, and attentive. We are so thankful to have found such a wonderful place for our mom. Having her in their care is very comforting! We feel confident that she’s in great hands! They go above and beyond to ensure residents (and their families) feel comfortable, engaged, and valued. The activities are thoughtfully planned and tailored to different interests, helping residents stay active and social. The facility itself is very welcoming. Communication is excellent—they keep us well-informed and are always responsive to questions or concerns. Truly a wonderful, kindhearted community! Thank you Beachside, Marlene and Kirsten! You are phenomenal and we are beyond grateful!
