Badger Prairie Health Care Center in Verona, Wisconsin offers 24-hour skilled nursing care for residents requiring long-term care or subacute services. The facility provides comprehensive person-centered and goal-oriented services designed to meet the physical, social, intellectual, and spiritual needs of residents.

Medical care is provided by UW physicians, with skilled nurses, MDs, and psychiatrists on staff making weekly rounds. The care center addresses complex medical needs including wound care, pain management, rehabilitation therapy, enteral feeding, hospice care, and behavior management. A consulting psychiatrist evaluates each resident individually to develop personalized treatment plans.

The newly constructed building, which opened in February 2011, features twelve households, each with its own dining room, sunroom, and living room. Residents have access to private bedrooms, private bathrooms, and outdoor patios. Four households provide specialized care: two with delayed egress and two locked households for residents requiring a higher level of supervision. Nursing support areas are positioned discretely throughout the facility to maintain a home-like environment.

Recreation Activity Therapy programs focus on enhancing physical, mental, emotional, and social functioning through leisure experiences. Dementia-specific programming includes sensory stimulation, aromatherapy, and reminiscing activities. Community outings are regularly offered, including pontoon boat rides, fishing trips, visits to farmers markets, restaurants, and movies, as well as bus rides and other excursions. Programs are available in individual, small group, and all-facility formats on a daily basis.
