My first visit on the premises was in July 2026 I'm impressed by the check in process in my opinion it serve as a secured way to moniter all visitors and protect the residence at the same time. The staff friendly and helpful I witnessed residents enjoying their workouts in the gym.

I am deeply saddened by the passing of my father. During such a difficult and emotional time, I expected compassion, empathy, and professionalism from the Director of Nursing, Mr. Albert. Unfortunately, this director showed no remorse, sympathy, or compassion toward me or my family. He was unable to answer my questions or address my concerns appropriately while representing the facility. Losing a loved one is already heartbreaking, and the lack of empathy and support made this experience even more painful for my family. Based on my experience with this director and the way the situation was handled, I would not recommend this facility

I would 5-star Azure Rehab except on the few days leading to my husband's release he had contracted another UTI which the nursing staff did not notice for at least three (3) days which is unacceptable. They bathed him everyday and should have noticed. The stench in his clothing and his diaper was awful. I did notify the Social Worker and Unit Manager of this error via text. After discharge from Azure, he was transported, the following day to the VA Hospital for treatment. I would add though, before this time, I was very pleased with the service. Joe was always clean, his bed was clean, the room was clean and the staff were exemplary; Shanita, Kim, attending nurses, Susie, Matthew and others, the therapists and their supervisor, Ben were exceptional. The desk clerks, even down to the janitor were friendly, professional, and helpful. All of the names, unfortunately, I cannot remember but I would recommend Azure to the community for the care of your loved ones.

I recently visited my mother-in-law at this facility and was extremely disappointed with the conditions and level of care. The entire building had a very strong urine odor, which is unacceptable in a place responsible for the care and dignity of elderly residents. I requested a pillow for her during my visit and never received it. It may seem small, but basic comfort items should not require repeated follow-up. More concerning, she has been waiting over a week for a basic evaluation. That kind of delay raises serious concerns about responsiveness and oversight. I was also disturbed to learn that residents are often encouraged to wear diapers rather than being assisted to the bathroom. Preserving dignity and providing proper assistance should be a priority, not convenience. Families trust facilities like this with the care of their loved ones. Cleanliness, timely evaluations, and respectful treatment are not luxuries — they are minimum standards.

Azure Shores is an amazing facility with an incredibly warm and welcoming environment. The staff is not only friendly, but genuinely compassionate and attentive. They take wonderful care of their patients, treating each individual with dignity and kindness. It’s clear that patient well-being is their top priority.
