As a person who has worked in health care I just want to say that this nursing home is more like a Death Home!!! So many things going on and not going on in the Woodbine, Iowa Azria Health Rose Vista! One of the major things is they do NOT check on their residents for hours. One resident was on the floor for TWO HOURS before they came to check on him! TWO HOURS!!! There are aides who enter residents rooms spewing filthy language at the residents. Theft is a big one! Residents personal clothing and belongs "going missing". One resident is missing all his Harley Davidson Shirts but everything else is accounted for. Sounds fishy doesn't it? The manager is a lair. He will lie right to your face and does not care. The patients rooms are disgusting and filthy. They have a cleaning lady who cleans 3 times a week and doesn't do much but visit with friends. The residents personal restrooms smell of urine and feces and are so nasty. The floors rarely get swept and mopped. The food is not suitable for human consumption. They do not always feed their residents on a daily basis either. They just say, "oh well we didn't think he was going to eat today". Why is that? Because you left him on the floor for two hours and he got a hematoma on his head so you think he should just starve now? The icing on the cake is they could not regulate a residence oxygen correctly so she died! One day she is just fine and the next she is dead because of their negligence. What in the world is going on? Apparently, not much but collecting a bunch of money for their pockets. If you want quality care DO NOT put your loved one here!

My dad has been here for about 3 months now, unexpectedly, the staff has been absolutely amazing with him! Have made him feel right at home! As a visitor, it's always Pleasant whenever you walk through the doors! Everyone is so helpful and smiling...it's always nice to visit!! Thanks for your good care of my Dad, Ken Gebel ❤️❤️

I have a long drive to go see my dad when I can get out there so it’s really important that he’s up and awake ready for a visit. If I call in advance, the nurses are always nice enough to make sure that he’s up and dressed and ready for a visit. It’s not easy for them because he’s unable to get himself out of bed and needs a big man. I appreciate all the efforts that they make so that we can enjoy our visits.

I am very pleased with Rose Vista. Mom has settled in and is adjusting well. The care and communication has also helped me adjusting to her needing to be there. The facility is nice as are staff members. Activities and encouragement are offered daily even though Mom chooses not to participate. I am grateful for the care she receives it gives me peace of mind between visits. Blessings to staff & residents!

My mother has been a resident for nearly a year. The staff is extremely caring and some of them have gone above and beyond what is needed. My father now shares a room with her there and they have a spacious double room with a private bath and shower. I visit nearly every day so I see how they are and give many thanks for their giving and caring staff. They have activities daily during the week and movies with popcorn on Fridays. There is a church service every Sunday. We are so fortunate to have a facility like this in our town.
