Avir at Town Creek is a skilled nursing and long-term care facility in Palestine, Texas, licensed for 102 residents. It provides post-acute care and ongoing support for individuals requiring 24-hour medical attention.
Family experiences at the facility are sharply divided. Several families praise the staff's attentiveness and dementia care approach, noting that caregivers "really KNOW each resident's needs" and work with dignity rather than over-medicating. These families highlight specific staff members by name and describe genuine compassion. However, other families report serious concerns: delayed response to call buttons, inadequate hygiene and turning protocols leading to bed sores, missing personal items, unresolved billing issues, and staff described as unhelpful or rude. One family wrote, "they take forever to answer a call," reflecting a consistent theme about responsiveness.
The facility holds a 3.6 out of 5 Google rating based on 20 reviews. Five-star reviews emphasize cleanliness, a positive atmosphere, and caring individual staff members. One-star reviews cite systemic gaps in basic care oversight and communication. The stark contrast suggests inconsistent care standards or significant variability in resident experience.

This facility was simply incredible. Very clean, great atmosphere. Staff was professional yet seemed to genuinely care about the residents as well as employees. Residents and employees appeared to be very happy and in good spirits. Can't say enough good!!!!

I put one star because my interactions with my brother's nurses have been pleasant. However, he still has skin cancer. They have tried to bill me for charges that are not my responsibility. Be careful with your loved ones. Pleasant nurses do not necessarily mean good care is being given. UPDATE: the skin cancer has just begun being treated. Also, I was told they are keeping his money.

Now don't get me wrong, there were a few, (very few) great ladies but overall this facility is absolutely terrible. They're always short staffed, and they take forever to answer a call. My boyfriend was sent here because they were the only facility within a 20 mile radius that would accept him. He suffered a stroke and was there to get better and for therapy. They take forever to get you changed, he went weeks between each bath which is unacceptable. His clothing came up missing, and they have a terrible therapy program. I would definitely recommend sending loved ones to a different nursing home/rehabilitation facility!!!! He's now in a much better facility, and definitely an excellent therapy program here. When he left he told them to make sure he had all of his clothes, they didn't, he had very little of his clothing with him!!!!
