Horrible security- let in unauthorized people several times. Left a stage five dementia patient outside unattended. Had the wrong name on medication bottles and administered at the wrong times. Went against Family, POA and doctors wishes regarding health and safety. Then lied about it. More than 50 violations in three months. Management is manipulative and vindictive and rude. There are a couple staffers there that are extremely kind, but they don't work nearly enough. Having an RN there only three days out of seven is a little scary.

We had our stage 5 dementia LO there for just three months. In that time we had dozens of health and safety Violations. When we brought it to the attention of the Leadership team, They wrote it off and told us they’ve “always done things that way.” So we went to the doctor and Pharmacy, and they were both appalled and even had to write letters to the director, informing them of their need to get things right. After that, the Leadership team Became manipulative and vindictive towards the POA They had the residents name wrong on her medication for three months. They administered medication at the wrong times. They let her go outside unattended They went against family and doctors wishes For her healthcare. They failed to chart all violations committed by staff that were called out by Family. Their security was absolutely awful, letting in non-authorized people several times. Once our LO was removed, they lied in their charting and was directed directly at POA conduct instead of the Daily routine of the resident. When time to fill out the tax form for the following year, they still had her name wrong, Left out an entire month of rent and were late sending the forms by a month. Took almost 60 days to return security deposit. Worst experience of our lives on top of an already tough journey . Please do some serious investigating when finding a facility for your LO. And stay far away from Autumns Promise. They will only promise to make your experience a living hell . I wish we could leave a negative rating. But I guess one star will have to do. On a sidenote, I just want to thank the handful of employees that were extremely good and kind and patient- but management needs to be revamped . And bribing good reviews with a free TV is pathetic.
