Arizona State Veteran Home - Phoenix

Arizona State Veteran Home - Phoenix (ASVH-Phoenix) is a 104-bed, Medicare-certified skilled-nursing care facility for eligible Arizona Veterans, located in central Phoenix on Silvestre Herrera Way, named for WWII Medal of Honor recipient Silvestre S. Herrera. The facility is state-owned and operated.

The home is divided into long-term care beds, a memory care unit, and a Medicare/rehab-focused unit, allowing it to serve a range of nursing and rehabilitative needs for Veterans.

Rooms are offered in single or double occupancy configurations, each with an ADA-approved en suite bathroom, a free private telephone, a television, free cable, and a DVD player. Facility-wide WiFi is available so residents can use their own personal devices.

Veterans and their spouses who choose long-term care are supported with customized services designed around their needs, with an emphasis on comfort, dignity, and social connection through activities and outings involving staff, fellow Veterans, and the broader community.

Additional amenities include a movie theater used for special screenings and documentaries, along with an onsite Veteran Benefits Counselor who assists with Veteran claims and benefits. Staff at ASVH-Phoenix provide nursing, therapeutic recreation, and social services for those who have served in the military.

Admission is available to honorably-separated Veterans or their spouses who require skilled nursing care as determined by a physician, with documented evidence of the absence of tuberculosis required as part of the application process.
