I have family that is there and I went to drop off some things to her. As I was walking through, the scent of urine was very prevalent. I also seen multiple elderly patients near falling out of their beds. Staff walking down the halls staring at their phones and not concentrating on where they are going. My family member said her food is cold when she gets it. She has to hit the help button repeatedly just to get someone to come help her eventually just to go to the bathroom. Staff is very unfriendly for the most part. As I was leaving, I could not get the door open amd was met with a rude response. This place needs to be fully restaffed with actual caring professionals.

Terrible conditions signs of water in wall by heating and cooling units. Signs of water damage in corner of walls and baseboard . Bathroom floors horrible from not being swept and mopped. Toilet set unhealthy when you lift seat you can see dried up urine . Windows has not been cleaned and miniblinds need to be cleaned . When you walk down the hall ways you see streaks all over from floor cleaning machine, no one mopping behind machine getting excessive water. Everywhere you go in hallways you smell urine. Most of all you can tell them about things they are doing wrong with patient and it goes in one ear and out of the other. Nursing Home means home for patient , not a free ride and easy money

One Star for smelling good only when you walk in! My sister’s experience here was absolutely horrible, and I would never recommend this facility to anyone who truly cares about their family. On her very first day, she arrived at 8:30 a.m. on a Friday and sat in her room the entire day without a wound care nurse ever coming by to check for sores or bruises. That was the first red flag. To make matters worse, she was placed in a room with a roommate who kept her television extremely loud all day. The rooms themselves are small, outdated, and uncomfortable. The food is terrible—truly some of the worst I’ve seen in a healthcare setting. The staff does the bare minimum, and when it came to therapy, there was no motivation, no encouragement, and no push to help her improve. The overall attitude of the workers was concerning; many walked around as if they didn’t care or didn’t want to be bothered. One nurse in particular had such a poor attitude that it made me question the level of training and professionalism in this facility. The worst moment came on the day of her discharge. Not a single nurse came to her room to assist. I had to walk to the nurses’ station myself just to let them know she was leaving. No one provided a wheelchair, so I had to roll her out of the facility myself. That was beyond disappointing—it was unprofessional and disrespectful. This place needs serious improvement in staff training, patient care, and basic compassion. I would not trust them with a loved one again.

I have been here a year and a half! The physical therapy and restorative care are very professional and caring! The rooms are very nice! The activities and activity director are very fun and helpful! Her assistant is very helpful and fun also! Everyone is welcome! The Administrator is very hands on and will always answer your needs!

On, July 24, 2016, my father died at the Woodlands Rehabilitation Center in Clinton, MS. I knew, the minute I walked into that place(Upon his arrival) they were not fit to care for my father. There were people in and out of wheel chairs(At least 30 wandering the hallways with loud noises). He was placed in a room where the other patient was out of his mind, screaming, talking to him loudly). My father was sick and I felt the hospital(Dr Alon, Baptist Hospital in Jackson, MS, released him from the hospital too early. We met with her on a Thursday. She stated she was sending him to rehab as early as Monday. So, we went out looking, trying to secure a place for rehab. She released him within the hour after us meeting with the first rehabilitation center, Woodlands. We felt we at least had til Monday for his body to strengthen. THIS PLACE WAS IN NO WAY EQUIPPED TO CARE FOR HIM.The Woodlands' was informed by Dr. Alon of his weakness. He needed immediate care and was not seen by the Dr. for hours without his feeding tube and other essentials. My father died within 36 hours of being in that facility. What upsets me is the nurses EXPLAINING he died in his sleep. I could tell they were not being truthful, especially, Ms Ramsey. My dear father was lying there, with his hospital gown on backwards. From June, when he was admitted, I stayed with my father at least 80% of his time hospitalized. I asked the nurse, "Did he not get up to use the restroom around 2:30-3:00 PM(Each night like clockwork, he'd attempt to get up which was prohibited for his safety). The nurses would come in and consoled him, explaining the safety precautions needed. My father was a gentleman and respected everyone(The good old boy way). She stated, "No, he has been asleep, since I came to work at 12:00 AM". According to the records, this did not happen. On August 24, 2016, my mother received a call from an employee stating they were there when it happened and she could not hold in what happened any longer. She states 2 men came in and mishandled my sick father by improperly restraining him causing a heart attack, whereas, he died. I am needing proper records and videos of what actually happened. The records sent to us have not corresponding to the Nurses story that night
