I debated about posting a review but Im honestly heartbroken. My grandfather age 91 living on his own, driving still, and a caregiver with no serious health issues went to the er for stomach pain on January 8th. After some test and scans he was found to have high blood sugar and fluid in his lungs but thats it. By January the 17th was strong willed and refused rehab and walked 25ft without a walker or cane to show the doctors how determined he was to go home. By January the 22nd and currently back home he was feeling like a bit of a burden so decided to go to willowcreek rehab for 10 days to get help getting back to himself. He was in willowcreek by the 23rd, he was supposed to have help getting strength back to walk better by himself again. February 6th still at willowcreek and family witnessed aids dismissing him about walking and shrugging it off, as well as sweeping around his food, and complaintsof him sittingin his room cold. Along with his appetite decreasing by the day, we were concerned. He was scared he wouldn't make it out of that place. Feb the 10th willow creek was called with our concerns and wanting him sent to be checked at the hospital but was dismissed because its not a medical emergency. By Feb 14th willow creek calls us his vitals are dropping sending him to er. At er blood clot is found in lung and leg, kidney failure, phenomena in lungs, and infection from sitting in dirty diapers to long without care. Feb 16th to Icu, and by Feb 20th on comfort care and passed away that afternoon. I do not blan all on willowcreek, but that man deserved so much better than how he was treated. Just please be aware of who's caring for your loved ones....

I would like to SHOUT OUT the absolute best service our mother received from the awesome, amazing nurses aide Michelle Mills‼️ Her warm, gentle, and pleasant work attitude was expressed through everything she did for mom, wish we could’ve had her 24/7 with mom. ALSO: the SW, Ms. Leigh Panashy, not sure if she’s the ONLY SW at this facility; but I feel she went above and beyond infinity to help my family throughout the mazes of Insurance issues, eligibility issues, and moving forward with providing healthcare at home for mom. PLEASE acknowledge these phenomenal women who have worked so diligently with our mom and family. I will be happy to speak with anyone regarding the exemplary care we received from these employees. Sincerely grateful, Shelby

Had family members here and they all had positive experiences. Don’t know what we would have done without Mrs.Mathis!!!!! She was very helpful in assisting us. She gave us a sense of comfort during the process. The therapy program was very beneficial to my family member. CNA’s went above and beyond our expectations.

My mom was at Willow Creek for several years with dementia. The staff were very kind and took great care. When she passed, the nurses were very supportive to me and told me stories about her. Mom was very well liked by the staff and I could see when I came to visit that she liked them. I'm grateful for the care she received.
