If it was so great, why do people score it so low. My guess is because it isn’t clean, very much in need of repair, rooms are small and they don’t notice changes in patients that can get out of control. You get what you pay for. I wouldn’t keep anyone there

I had to give it a one star only because 0 isn't an option displaces. Disgusting please do not send your family to this place. Keep them at home at all cost Bedbugs, toilet full of mold patience sitting in the same clothes.They'd worn for 3 days or more staff, very unfriendly and not very cooperative. I'm telling you, this place is the worst of the worst. Please do not send your family members here and spread the word

I went to the pysility to visit my brother who is still currently there. If you care about your loved ones DO NOT SEND THEM TO TRI STATE!! Please. When I arrived I spoke to my brother he stated he hadn't had a bath in a week. One time he had a wet bed he turned on his light , he layed there for over an hour, when I arrived I went to the nurses station it took him additional 20 minutes to get there to help change him and his bed. He was supposed to have restorative therapy and they said he would have it 3 to 5 times a day. We saw them once that week, she came a second time and said she would be back. We didn't see her again, case manager was rude. We talked to the nutritionist and he was supposed to have a nutritional shake with lunch and dinner. He had 2 in a week and didn't get anymore, they haven't helped him at all!! They are supposed to get him up for breakfast and because he can't sit in the wheelchair for long periods of time. They stopped getting him up, tri state is absolutely HORRIBLE!!! After posting this review I talked to my brother and he said that things got even worse. The some staff said "what does he expect, Champaign treatment on a beer budget" my brother said they treated him even worse. Not answering his call light for up to 2 hours, not helping him get out of bed. They treated him even worse than before, I didn't think that was possible. Most of the staff are horrible, their attitudes are horrible. There were a few standout employees, but the horrible ones outweigh that. Staff 0 stars, horrible place!!!! And people!!!

TRI-STATE REHABILITATION and NURSING is the BEST facility mostly due to the EXCELLENT Administrator doing great job! I had experience searching for the right place for Mom and Dad. Tri-State can't be beat. Thanks again for looking after them. Sincerely, Melinda Lau Recently I have read numerous complaints about various nursing/rehabilitation facilities regarding upkeep of buildings and cleaning, (eg shower areas, floors, etc). Machines not working, (eg clothes dryers). Some residents not taken care of properly and other problems. TRI-STATE still great!

My mother was put in this facility from a referral for Hospice Care. Understanding no facility is perfect! But the care my mother has received so far has been great. Lynneice the DON very friendly and welcoming. Always responds to my calls or texts in regard to my Mother's care. If any issue does arise, she is on top of it. My mother has been doing well and states she likes the facility. I would highly recommend Tri State if you had to put a family member in a Nursing Facility.
