Being a nurse, the standards of excellent care are held to the highest, and what myself and my family experienced at Sunset home on 4th floor Riverview was anything but, it was neglect. Our mom has been in and out of Sunset, and was in assisted living, and then rehab off and on. The care that she received in those places were exceptional. The 4th floor Riverview unit needs to be closed down. Mom is declining in health, and will be going on Hospice, and when she was transferred to this unit, it was a shitshow from the moment she arrived. Vitals were taken, and we didnt see anyone until 3 hours later when she was turned. Call light was attached to her bed cover, of which she was unable to use. The bed was pushed up against the wall with mom's catheter attached. No nourishment was offered to mom since she arrived. A bug crawled out of her bed and onto my leg. The room was disgusting! The DON was informed, and she said that she would address the issues in the morning-unacceptable! Our mom is now back in Blessing, where she has been receiving the exceptional care that she deserves! Terrible and unsafe place for anyone who is there long term. My mom was on the rehab floor multiple times and in assisted living for a few months & received good care. After a stay at Blessing, she was going to be placed on Hospice and transferred back to 4 River at Sunset. She arrived by ambulance on 2/26, the nurse came, took her vitals, did not speak to mom or the family & just left the room - we didn't see anyone again for 3 hours. The bed was shoved up against the wall, bugs were crawling on the bed and floor, the call light was placed in our mom's hand even though she was unable to push the button for help. There was a resident wandering into residents rooms, including my mother's. The dining area had leftover food on tables and the floor and bug traps placed on the floors. Jenn with Social Services came up and said that she would get maintenance and a bug man up to take a look, nobody came. Our mother was non-verbal at the time and bed bound - she was unable to advocate for herself. THIS WAS NOT A SAFE ENVIRONMENT FOR HER OR ANYONE ELSE! We told the nurse to call an ambulance because we wanted mom transferred back to Blessing. She call her DON to let her know what was happening and that the family was very upset, the DON's response was "I just left and will deal with it tomorrow". Placing a loved on Hospice is such a difficult decision to make & Sunset should have been more attentive and empathetic to the situation. I feel horrible for anyone who is there because they are not receiving the love and care they deserve.

Short staffed. Residents roaming into other rooms. Mice waste in residence bed. There are caring people there but, they are overwhelmed.

Due to labor law and Osha standards they need to keep a phone line open if I or any human resources department employee ever call on the behalf of our spouses or otherwise have the silent answer and hang up put to us three time in a row again it will be reported
