My aunt was admitted to Sonoma Post Acute for rehabilitation, and I can’t say enough good things about the care she received. The therapy team did an outstanding job helping her regain her strength and confidence. Thanks to their dedication, she was able to walk confidently again and return home before her insurance benefits ran out. My aunt wasn’t the easiest patient, but every member of the team treated her with patience, compassion, and respect. I want to recognize not only the therapy department, but also the nursing staff, activities department, and social services for working together to provide exceptional care. From the moment you walk into the building, the staff is welcoming, attentive, and genuinely caring. They even greeted me by name, which made every visit feel personal and reassuring. If my aunt ever needs rehabilitation again, Sonoma Post Acute will absolutely be our first choice. I highly recommend Sonoma Post Acute to anyone looking for a skilled, rehab facility.

A few great CNAs like Jose, Blanco, Janet, Axel, Maurice, Pedro, Ofelia, the nurses are all great & so is the PT Dept. The staff at Sonoma Post Acute is great very loving and caring. I will miss you all 💕

Our family is incredibly grateful for the exceptional care our 84-year-old mother received while recovering from a broken hip at this facility. From the very beginning, the admissions department went above and beyond to help coordinate what turned out to be a complicated transfer from an acute care facility. Their professionalism, persistence, and compassion made a stressful situation much easier for our family to navigate. We are especially thankful for their efforts in ensuring a smooth transition and helping get our mother the care she needed. Throughout her stay, the nursing staff, therapists, and support personnel treated her with kindness, dignity, and respect. Recovering from a hip fracture at her age was no small task, but the team encouraged her every step of the way and helped her regain strength, mobility, and confidence. A special thank you to the physical and occupational therapy teams. Their skill, dedication, and genuine concern for her well-being played a vital role in her recovery. They kept us informed, answered our questions, and celebrated every milestone with her. Entrusting the care of a loved one to others is never easy, but this team gave us peace of mind knowing she was in capable and caring hands. I would like to start by thanking the staff at Sonoma Post Acute for taking such great care of my mom. She was a patient there for quite some time before transitioning to an assisted living facility. A special thank you to the therapy department for working so hard to help her return to her baseline. Your dedication truly made a difference. I would also like to recognize the social worker, JC, who was wonderful throughout the transition to assisted living. They made the process easy by communicating every step of the way, arranging home health services, helping find a new primary care provider, and ordering necessary medical equipment. Thank you as well to the nursing staff and CNAs for the compassionate care you provided to my mom. We are very grateful to Sonoma Post Acute for providing such excellent care.
