My father was neglected at this facility. They lost all his clothing and he back tracked 100% from the time he discharged at CMMC three weeks ago. He was walking on his own at the hospital now can barely walk without a walker after 3 weeks of being there. My family was verbally abused by a nurse named Michele and all the feedback we gave fell upon deaf ears. When we showed up today my father was in someone else’s clothes with his pants around his ankles attempting to urinate alone. He has a hx of UTIs and although he had been confused for the past week, incontinent and not well at all plus his hx of UTIs and them causing seizures was all through his records we were not informed about any of these symptoms and the infection has only gotten worse. Being in the healthcare field I have never witnessed the neglect and such an awful LOC anywhere in my life. If you want your loved ones to get help and actually recover, do not send them to this facility. The facility smells like urine as soon as you enter, there are patients spitting and falling in the hallways and nobody seems to care or help. I’m absolutely appalled by what they have done to my father, the cleanliness of the facility, and the sheer lack of care or empathy they provide. Nurse Michele should never be allowed to ever work in healthcare again. What happens to clients who don’t have someone to advocate for them. I’ll tell you what, they are not rehabilitated and end up in a wheelchair probably at the nursing home connected to the facility. I’m saddened by all this and can only hope this helps someone else’s loved one get the care that is needed.

The staff that care really do but a few bad apples ruin the bunch. Food leaves alot to be desired and lots of mistakes with diets and with trying to get likes/dislikes. College level dorm cooking at best. Overwhelmed staffing seems to be a COVID-19 aftereffect never enough staff and the rush to fill beds

My sister in law has diabetes and has received excellent care. She has been happy there and we are so thankful to each of caring staff.

We placed three dear friends at Pinnacle and have been nothing but happy with the care they continue to receive. The Staff are very helpful to us with all of the paperwork required for each one and patient! We are thankful that they're in a Facility that has caring Staff.
