Pillars of Care Assisted Living Home

The admission process includes an initial conversation to understand the individual's needs, a comprehensive assessment by the home's RN or administrator evaluating medical needs and functional capacity, completion of resident agreements, and a care conference involving the resident, their representative, the Pillars of Care representative or RN, and necessary medical team members to develop the resident's Plan of Care.
