I recently had the opportunity to work with the team at Phoenix Mountain Post Acute, and I was very impressed with the level of care and professionalism demonstrated throughout the facility. The staff consistently treated residents with kindness, dignity, and respect while maintaining a welcoming and supportive environment. Communication between departments was strong, and it was evident that the team works together to provide quality, patient-centered care. The nursing staff was attentive, the therapy team was encouraging, and the social services department made every effort to support residents and their families during transitions. The facility was clean, organized, and focused on creating a comfortable atmosphere for residents. It is clear that the employees genuinely care about the well-being of those they serve and strive to make a positive difference each day. I would confidently recommend Phoenix Mountain Post Acute to anyone seeking compassionate rehabilitation or long-term care for themselves or a loved one.

This facility is such a great care center.<br>My mom was needing therapy before going back home because she became so weak from her lengthy hospital stay. After a long stay at the hospital, she did NOT want to go to a skilled nursing facility. However, Phoenix Mountain exceeded all of our expectations. The therapy team go her up and running again and I felt the whole team was involved in her care from the start! Karina was our case manager who was so kind and knowledgeable and really helped us through the process. I highly recommend this facility to anyone needing nursing facility care.
