Passion Adult Foster Care

Passion Adult Foster Care is an adult foster care community in Coon Rapids, Minnesota, serving individuals 18 and older who need assistance due to age, disability, or other life circumstances. The community provides supportive living arrangements with 24-hour care, including personal care assistance with activities of daily living such as bathing, dressing, meals, and medication management.

The community serves individuals with a range of needs including intellectual disability, mental health conditions, behavioral issues, autism, chronic illness, hearing loss, learning disabilities, memory loss, physical disability, and vision loss. Services include individualized care plans tailored to each resident's medical, emotional, and social needs, health support with coordination of healthcare providers and appointment assistance, and community structure and safety.

Passion Adult Foster Care occupies a spacious house with three large single bedrooms and two roomy living rooms equipped with cable television and internet. Each resident has their own private bedroom with HD TV and entertainment. The facility includes accessibility features such as wheelchair access, Hoyer lifts, ramps, hospital beds, and hand rails as needed.

The community emphasizes creating a nurturing environment that fosters independence and well-being. Staff provide assistance with maintaining clean living spaces, coordinate transportation for medical appointments, social activities, and community events, and encourage family involvement through open communication. Services are delivered in accordance with DHS policies and procedures, and the community accepts funding through DD Waiver, CADI Waiver, TBI Waiver, Elderly Waiver, Medical Assistance, and private pay options.
