This place is horrible!! Smells really bad anytime I come morning,noon or night! I found my aunt who I was visiting on the floor in her room. There was MO sense of urgency to get her up. I told the nurses at the nurses station she needed to use the bathroom twice.. I wait 40 min and left because no one ever came and I was so upset! Do not come here even if it is a last resort!😢😢😢🥵🥵

My friend was in here not sure if he still it because he went to the hospital and was there for a while I called the hospital and they said he wasn't there anymore, I asked if he went back to the nursing home were he was before and they said that they don't know, so I called a few times and every time I call no one answers so I have no idea if he went back or not

My sister stayed here for a few months. For what it is I thought it was decent. Nursing homes can get a bad rap and sometimes for good reason. This place was fine but the best part of the place was the physical therapist Pinal. She was outstanding! When I met her I felt much more comfortable leaving my sister there. She really seemed to care about her job. She was determined to help my sister improve and get back on her feet. Incredibly she still calls me to see how she is doing. I honestly never expected the type of care that she provided. Hopefully you will be fortunate to get her to take care of your loved one.

This place is terrible. It's not a rehab center. It's a holding pen. Think "One Flew Over The Coocoo's Nest." It's terribly run down, dirty, and smells just the way you think a run down, neglected nursing home would smell. There is high personnel turnover. I extracted my sister from the Park Ave facility where I'm sure she would have deteriorated. Several reviews on Yelp attest to the terrible care at this place. Don't be fooled by a few OK reviews here on Google. The place should get zero stars.

I was recently visiting family that just transferred to this facility,I first had thought because of the reviews that there was going to be issues, but after arriving I could see right away that the reviews are absolutely wrong. The nurses and especially the director of nursing Juan were all very hands on with the patients to make sure they felt comfortable and like they mattered. I would also like to say that the activities they played had my family members very excited and they all joined in to play jeopardy.5 ⭐️
