About Nature Trail Health and Rehab Center

Nature Trail Health and Rehab Center is a 74-bed skilled nursing and long-term care facility in Mount Vernon, Illinois, serving patients who need rehabilitation and ongoing medical support.

Families frequently praise the warmth and attentiveness of the staff, with several noting that caregivers treat residents with genuine compassion and go beyond typical care duties. The Activities Director is singled out for creating a fun, engaging environment. Leadership is mentioned as hands-on and dedicated. However, reviews reveal significant operational concerns: multiple families report lost or misplaced personal items, medication errors, and inconsistent assistance with meals, one family noted waiting over an hour for help eating, leading to substantial weight loss. COVID protocols were also cited as mismanaged in at least one account.

The facility holds a 4.4 out of 5 rating across 58 reviews. While many families describe exceptional personal attention and a welcoming atmosphere, the pattern of medication mix-ups and inadequate support with daily activities suggests variable quality in day-to-day care delivery.

Our family is incredibly grateful for the care my mother received here. The staff didn't just provide medical support; they showed true compassion and empathy every single day. They were always available to answer our questions and supported our entire family through a difficult time. It is rare to find a team that treats residents like their own family, and we cannot thank them enough for their dedication.

I appreciate how well we treated upon arrival with my mother. We were scheduled to go somewhere else and that other location was horrible. My father was here in 2016 and he was treated very well. Everyone there did everything they could to welcome my mom and help her be comfortable. This is never easy but they certainly made the process easier. They even understand that I’m trying to get her closer to where I live and want to help me accommodate that as well. What a great group of compassionate people.

Everyone was pleasant and helpful. They seem very upbeat with the residents. My nephew has been there for a while. He likes the people there and tells us that they treat him well.

Summer never disappoints! She is extremely aware of her surroundings and always there to help before being asked. I love visiting when she is working my dad’s hall.
