Natchitoches Community Care Center

Natchitoches Community Care Center is a skilled nursing and rehabilitation facility in Natchitoches, Louisiana, offering transitional care following illness or injury as well as traditional long-term care. The center operates under a person-centered care philosophy that places residents at the focal point of all operations and design decisions.

The facility is organized into four households, each with its own kitchen, dining area, living area, and outdoor space. Private resident rooms with private bathrooms surround common areas, and each household has a front porch for outdoor enjoyment. Staff are dedicated to individual households and are encouraged to dine with residents and participate in activities such as cooking favorite dishes, fostering an atmosphere where staff become extended family members.

Rehabilitation services include physical therapy, occupational therapy, and speech therapy provided by fully equipped staff in a dedicated rehabilitation gym. Many residents complete rehabilitation and return home successfully, though the center also provides long-term care for those who need it. The facility works toward helping each individual function at their highest level of capability.

Residents have access to a range of amenities and activities, including a café offering coffee, beverages, and soft serve ice cream, as well as salon services and fitness centers. The center offers trips into the community and life enrichment activities, and families, community members, and volunteers are invited to participate in social activities. A library provides resources on topics relevant to elders and their families, including health, wellness, medical conditions, and information about Medicare and Medicaid benefits.

Established in 1994, the center is operated by CommCare Corporation, a Louisiana-based non-profit. The facility accepts Medicaid, Medicare, some insurance plans, and private payment. American Sign Language and virtual foreign language interpreters are available upon request.
