We feel incredibly lucky that my mom was referred to Mission Palms for rehab. We initially planned to stay two weeks, but ultimately extended it to two months because we were happy with her progress and our experience with the facility.
After an extended hospital stay, my mom was deconditioned quite a bit to the point that she could not get out of bed on her own and really struggled to stand up. At MP, she received almost daily OT and PT sessions with well-qualified personnel who were very kind, patient, and caring. I got to watch a few sessions and I was so impressed. Thanks to the great work that the OT and PT teams have done, she has gained a lot of her independence back. She can now do daily activities on her own and walk around her house with just a walker. Thank you Noelle, Long, Alyssa, Son, Nghi, and the rest of the teams. We appreciate you so so much!
The medical team here is great too. There were a few instances where my mom needed medical attention, and the nurses responded promptly and kept me in the loop. Her attending, Dr. Chan, is also wonderful. He was very easy to talk to, responsive to concerns, and very human.
The patient care staff that look after patients 24/7 are very sweet here. They were very friendly, very encouraging, and responsive to patient needs. My mom only speaks Vietnamese, so she was really appreciative that there were Vietnamese staff. Thank you Kelly, Loan, Linh, Kaleb, Son and many others that I didn't get to meet! You made her stay really comfortable and helped keep her sanity.
This facility can also arrange transportation for medical appointments, too, which we needed.
The place is kept clean. There are daily entertainment and special dinners every couple of weeks. My mom especially enjoyed it when Co Que made rounds with ice cream.
Kimberly Dao also followed up with us to ensure that we had everything we needed after discharge.
Overall, we are very happy with our experience and recommend this place if your loved ones need care or recovery.

Healthcare Center for skilled nursing and rehabilitation services. The entire staff from admissions, nurses, cha's, nutrition, physical therapists, kitchen, facilities etc. are very professional & quick to respond to every question and/or need that my husband has. My husband 's health is poor, but his mind is as sharp as a tack. He is extremely opinionated and won't hesitate to speak up to let me or the staff @ Mission Palms

Very clean and easy to access. The staff is helpful and friendly. Facility is well care with free odor and provided a lot if daily activities for patients. My Mom is very happy and well taken care by staffs of nurses and care giving . 👍 Good job guys and keep it up

It is clear that the teams providing patient care went above and beyond with respect to physical therapy, nursing, dietary needs, activities, social services, and doctor attention. The CNA's provided the most often touch points, and we couldn't be happier with how they treated us. The facility is very clean and inviting and had such a warm atmosphere even amidst the COVID-19 chaos.
The entire team, from reception to maintenance (yes, we had some requests which were attended to very quickly by the maintenance department), even the folks helping us with the medical records, was absolutely fantastic. If your family needs a nursing home, I highly recommend Mission Palms.
