I was so worried to leave my Dad at a nursing home . He was sent there to rehabilitate from major health decline after surgery. I couldn't be more grateful for how the staff at mission took care of him. They also took care of us, as they kept us up to date and informed every step of the way . We are very pleased with how my dad was treated at Mission. Everybody was amazing. The doctor was very informative and took time to explain everything to us. The staff was very helpful and attentive. The facility was clean and comfortable.

I don’t often leave reviews, but after seeing complaints about the carpet, I feel the need to put in my two cents. If you haven’t been there lately, the entire place has been renovated. The new flooring looks wonderful. The Mission in Nephi Has been an amazing place for my sister who is disabled. Lots of staff on hand, and they work like crazy! I know, because I am there often. I have gotten to know them personally. The physical therapy is wonderful there. They also have amazing activity people who keep her busy every day with new and exciting things. She just loves it. They have bingo, crafts, prizes, music events, karaoke, spa day. And If there’s anything that I want changed, they are happy to talk with me and work with me. You become family.

In Mission at Nephi Nursing and Rehabilitation I received excellent care and services. I stayed in Mission at Nephi seventeen days after a total knee replacement. All the staff, from Admisions, Physical Therapy, Medical, Nursing, CNA's, Recreation to Maintenance are very professional, empathetic and compassionate. Oh, well, they are all wonderful. My recovery have been fast and not as painful as I expected thanks to the care I received. I had physical therapy twice a day with two great physical therapists, plus exercises to do by myself. And, enjoyed recreational activities among the other residents. Lived those days in a clean environment, as the rooms are cleaned every day. The food was really good, and so were the snacks. My medications suited all my needs and were given timely by the awesome nurses that work 24/7. And, in a bit more than two weeks I was able to interact and observe other residents. It has a sense of community, with activities to fill the days, and good interactions. Between the friendly professional staff and sense of community, Mission at Nephi Nursing and Rehabilitation is the ideal place for rehabilitation after a surgery or for permanent residential needs.

Great staff and facilities! Recent renovations are a welcomed improvement. Admin team, doctors, nurses, assistants, therapy and recreational teams, kitchen staff, facilities team, etc. are all top notch. The residents are well-taken care of and their needs are met on many levels. I was able to achieve my goals of rehabilitating through the demanding care of everyone involved. Please put Mission on your list of options for a comprehensive and caring rehabilitation and nursing facility.

Please, please do not bring your loved one here. My dad broke his tibia and fibula in his leg and needed a safe rehab environment before and after surgery so he could heal properly. I truly believed keeping him close to home would bring our family peace, but the moment we walked into this facility we knew we had made a huge mistake. The building was dirty — honestly, filthy. There was a clear lack of the basic equipment needed to care for someone in my dad’s condition. My dad asked for Tylenol and was given oxycodone instead, which was extremely alarming and unacceptable. The transport ride to the facility gave him severe anxiety (and my dad is not an anxious person), and even the van was dirty. The bed was barely supportive, with a thin mattress that caused him significant back pain, and the pillows were worn and unsanitary. Nothing about this environment felt safe, comfortable, or appropriate for recovery. A bath wasn’t provided and there wasn’t a shower in his room. Just a broken toilet. My dad stayed less than 24 hours. We immediately requested a transfer because we knew he deserved better care and a clean, properly equipped facility. I could go on and on about how horribly disgusting this facilities is. I share this so other families can make informed decisions and protect their loved ones.
