At first it's great, but the facility sent my mother to the hospital for evaluation... Because her conditions stay in the hospital more than one week... The facility took advange to refuse accept my mother back... They are low in staff.... Very disappointed the facility.

My husband was admitted to Live Oak Rehab on July 18, 2025, following a right total hip replacement. Upon our arrival, we were warmly welcomed by Maly. I was initially worried that our insurance might not cover the services, but she reassured me and clearly explained that everything was authorized and covered by our provider. We were extremely satisfied with the care he received. Everyone—from social services staff like Gigi and Ruby, to Armando (RN), the physical therapists Allen, Ritz, Nancy, and the rest of the nursing team—was dedicated and kind. The facility is consistently well-maintained by the custodial staff. I would absolutely recommend Live Oak Rehab to anyone in need of a supportive and welcoming place for recovery.”

I don’t understand why this facility is not rated higher!!! My mom is there and the staff are incredible, accommodating, kind, attentive, professional, and knowledgeable. I have literally entrusted them with my mom’s life after taking care of her for 20 years. Since she’s been there, she’s put on weight (which she desperately need to) and maintained her weight, she is happy every time I talk or visit with her. Her disposition is excellent. The only reason, I didn’t give them a 5 star rating is because of a communication issue, which has now been rectified. If you’re looking for a place to take care of your elderly loved ones, this is the place!!!!! Kudos to Gigi for her wonderful professionalism.

I am very pleased to report that my 94-year-old mother had a highly positive experience during her initial three-day treatment at Live Oak Rehab Center. A significant decline in her strength late last year necessitated a three-day emergency hospital stay, which left her feeling discouraged. Thanks to the exceptional care provided by Alex, Chris, Alan, Armando, and the entire team, my mother has regained her confidence and enthusiasm for life. Alan's assistance greatly improved her ability to move between her bed and the bathroom. In addition, a nurse's thoughtful provision of denture adhesive has led to an improvement in her nutritional intake. =================================== This is a follow-up review, two months after the previous one. I reaffirm the five-star rating for this facility. Following her initial three-week rehabilitation stay, my mother reported significant improvement and expressed a desire to return home. However, the rehabilitation team leader, Alan, and the social coordinator, Gigi, appropriately advised that her recovery was incomplete and required physician approval for discharge. Following the physician's approval, my mother was discharged. Unfortunately, a subsequent fall resulted in a spinal fracture requiring surgery. She has therefore been readmitted to Live Oak Rehabilitation Center. As before, the facility was well-prepared for her arrival and provided excellent care. All staff members demonstrated exceptional courtesy and respect, creating a welcoming and familiar environment. My mother is highly satisfied with her care and progress. In retrospect, we regret not heeding the center's professional assessment during her initial visit.
