My mother was recently placed in there. My stepfather told me about the mistreatment that she receives. He brought it up to them,so they banned him from the facilities. My grandmother would bring her stuff and they would "mysteriously" disappear. If it was possible to find a better alternative home for alzheimers/dementia patients, we would gladly take her there. Never heard of or seen a place that was SOOO unprofessional and totally incompetent. But don't worry, the state attorney general, lawyers, and even the Lebanon police have been contacted. The people there deserve better

Most of the staff seems to be genuinely caring for the patients. The weekend charge nurse is very difficult to deal with. She works two eighteen hour shifts every weekend. Ii don't think you should be able to work 36 hrs in two days when you are in charge of giving. Out controlled drugs.

I would give zero stars if I could. DO NOT SEND YOUR LOVED ONES HERE. Mom was placed for rehab after a fall. We observed filthy conditions. She was sat in a wheelchair covered in rust, food, and vomit, and that's only what we could see. The tables were not wiped down, the floors were filthy. The smell was abhorrent. Look, I understand it's going to smell a little like urine, but this was indescribable other than to say it smelled as if hopelessness and death had a baby which they bathe in urine and allow to smear excrement on the walls. It was unusually bad. The food was nearly unidentifiable and had zero identifying flavors. Mom was sat in a wheelchair with no cushion for several hours. We were visiting her for 3 hours and she was not toileted once. No staff members asked who we were, only who we were there to see, and some locked doors had the door code printed above the keypad, so security is lax. Mom has some dementia but was placed in what looked like a long term severe mental needs ward for what was supposed to be a short stay. I felt terrible for the people there; no one should be forced to live in those conditions. We couldn't get Mom transferred out of there fast enough. Praying she's not got any pressure sores, Staph, or MRSA from her stay, will update once I hear back from the nurse at the new facility on her condition upon arrival.

Don't put your loved ones here, because they'll try to get them medically dictated as incompetent and get a doctors power of attorney over the loved one as soon as they decide they want to transfer facilities. Not to mention tons of neglect, just letting clients sit for hours in rooms with nobody checking on them and not getting meals for hours, if at all. Not just me has witnessed it but even a nurse from another facility. They will take a strangers word about a condition, instead of getting a brain scan done to make sure client actually does or doesn't have alzheimers/dementia, and will take someone's word even if they're lying. This place is not a place for someone you love who needs elderly friends and activities to do. This is a place you send them to rot and die. Oh also they have bed bugs! And yes I will report them and sue them to make sure my loved one has/gets what they need and to a proper facility they actually enjoy
