I had a stroke and was taken to John Muir and had five doctors working on me and ended up in ICU with no use of my legs. I discharged to La Casa Via Transitional Care Center end of December and have been going through the physical therapy program to build my leg strength and walk so I can return home. Physical and Occupational therapists and the RNAs have done a great job getting me stronger. Now one of the managers, Cam Jones, has been a godsend to me. Her drive and her dedication to her work and clients is inspiring. Cam has stepped out of her job description and put in the extra time and effort to use her expertise in this field to help navigate paperwork, and use her communication skills to arrive at the best outcome she could muster with her dynamic personality. She has helped with my taxes, medical forms, and helped with the papers needed to apply for Medi-Cal. I am discharging home today and would highly recommend Cam and La Casa Via Transitional Care Center to anyone in need of excellent rehab and support.

When your parent is vulnerable, you want to know they are in good hands—and that’s exactly what we found here. The staff treated my father like family, not just a patient. Their kindness, patience, and dedication meant more than I can express. This place gave us peace of mind during a very difficult time, and I will always be grateful for the care they gave my father. Special shout out to the wonderful physical therapist, nurses, CNA’s. Thank you Jean for giving my father a haircut it meant the world to him. I could not be happier with the care my mom got at La Casa Via. She suffered a hip fracture and we chose La Casa Via for two weeks of skilled nursing rehab. From the point when we started looking at options Cam and her team at La Casa Via stood out. They were extremely responsive and took time to understand mom's special needs and addressed them Once there, the special attention and care continued. All nursing and CNA staff were extremely responsive - day or night. And the physio and occupational therapy teams were also top-notch in helping her become mobile again. On discharge also they went above and beyond. Not only did Cam coordinate hand off to a local home health care provider but also loaned us some of their equipment including a wheelchair for a few days till we organized our own at home. I highly recommend La Casa Via to anyone evaluating rehab/skilled nursing facilities.

Okay, first of all, someone posting a five-star review of a skilled nursing home must either be a shill or insane, right? Read on. My father suffered a serious fall down a full flight of steps earlier this year, suffering multiple debilitating injuries. He was in the ICU for three days and a trauma ward for four. Prior to discharge, I had heard great things about La Casa Via, and after touring three other facilities (all of which either my mom or dad had been treated in due to other injuries or surgeries), my parents and I concluded it was the best option. Cam Jones was incredibly responsive from the moment I reached out. She helped get my father, who was in really bad shape, situated right away. The facility itself is very clean, but some of the furniture and equipment is old and worn. I think many would judge it to be less desirable than some of the nearby alternatives based just on the real estate, but that would be mistake. The floor is buzzing -- staff are helping patients all the time and the response times are very short. (In contrast, I recall a nearly two-hour wait for my dad to get pain meds in another nearby facility three days after surgery.) The nurses, nursing assistants, PTs, and OTs are all friendly and capable, and they genuinely seem to want to be there. Big contrast to other places. The place is clearly managed well. Finally, they helped my dad get from being unable to feed himself to being able to move to assisted living with a high degree of independence, mobility and strength. He still has a further journey to a full recovery, but the progress he made at La Casa Via is nothing short of miraculous. For that, I credit their awesome team. Hats off to them.

La Casa Via is truly exceptional when it comes to the staff. After my 82 year old mother broke her femur, the care she received was outstanding. Director Camalin Jones goes above and beyond her job description, personally catering to my mom's needs and making sure she felt comfortable and supported. The nurses, CNAs, and therapists were compassionate, professional and attentive. The rehab team helped my mom regain her strength with care and encouragement. I am so grateful-highly recommend!
