My mother was there and put on hospice and her hospice doctor had put her on comfort every 2 hours before she died and seeing how hospice doctors can no longer override the doctor of the facility they had to wait on the doctor to get back to them before they would give her the comfort meds every 2 hours the doctor would not even acknowledge the fact that anyone was trying to get ahold of him all he does is show up once in awhile to get his check from the facility he is a garbage doctor and does not care 2 cents for the residents thatlive there the only reason this place is still open is is because it is convenient for Ava Missouri for people

I was at heart of Ozarks in Ava mo for short term for my rehabilitation… I never have a problem they really friendly and really encourage me to meet my discharge goal… thanks to them … I was stay there for like almost 1 month … the rehab worker did amazing job they been encouraging me to keep goal and nurse people really nice to me … I love to pick on them to have a wonderful time .. I’m sure miss them

They serve cold substandard food. CNA lady came and screamed at me at 3.00 am. Charge nurse told me to leave because she would harm me. Because she hot lined her. Bathrooms are live sewers. Social work all she cared about was flashing her diamond that her husband is ruch l was not allowed to scelebrste my christian birthday.or have some one pick up bed pads. I was discriminationef for my faith. She took my food stamps for 3 days of cold food fir revenge

Very poor care for my husband….during Covid quarantine an elderly male patient wheeled in and out of my husband’s room, rummaging through his personal belongings, actually drinking out of my husband’s water glass! And then scolding my husband for being upset about this, saying that this gentleman couldn’t help it, he had dementia! At the very least this man should have been put into quarantine after being in the same room with my husband who was under quarantine himself! I’m very sure this gentleman’s family was not made aware of the situation! There was not one good thing that can be reported on this “Rest Home” defiantly should not be considered a rehab!

My dad's just been released from the hospital and spent 2 DAYS in this facility where they had no idea how to even monitor or operate his oxygen machine!!!! I hate leaving poor reviews but this could have easily killed him had my mother not went to see him. I'm highly disappointed with this place rn.
