I gave 2 stars only because there were a few great nurses and aides. However PT was in my opinion incompetent because they did nothing but complain how my girlfriend wasn't cooperating, wasn't trying. I explained that she is very sick and they need to be patient with her. They said they have policies and procedures. I told them everyone is different and rehabilitate at a different pace. I told them its their job as the professional to help her, to motivate her. They didn't care and didn't try. They reported to Medicare that she wasn't making any progress even though her condition had been stable for a couple weeks which was not true. Medicare wouldn't cover her after that and was sent home. She was home for one day and then was taken to the hospital by ambulance for 10 days and is now home on hospice. I guess she wasn't so stable after all. As for OT, I heard she was getting OT but never saw it, never even met the OT person so I can assume they weren't doing their job either. We will never know how much better she could have gotten had this facility, Social worker, nurse manager, anyone... gotten her the help that her and I both tried to tell them over and over that she so desperately needed. I wouldn't recommend this facility for rehab unless you or your loved one are very healthy and of sound mind to a point that your loved one can tell you everything that's going on there. PT was terrible!! Very unprofessional! I was there every day and saw plenty but I know there was so much that I didn't see or even get told because my girlfriend was extremely confused due to how sick she was. She also had large unexplained bruises on her back. My heart breaks for her! She deserved so much better.

I'm only giving a 1-Star because I cannot give anything less. A family friend is currently here, and is being mistreated and neglected. Has been receiving inconsistent care, and the staff is absolutely heinous in how they treat the patient. There have been broken bones, unexplained brusies, and when questioned the story changes. And to add to it, different staff members have different versions of the story- as in there are different stories being told about the same incident. They take forever to respond to calls for help, they deprive patients of their basic human rights, and are just all around nasty to those they are supposed to care for.

The staff always seem to be confrontational. There is a lack of communication that creates a quality of care. Food seems to be rationed out and each resident is treated as if they are in LTC vs. Transitional Care. When questioned about their actions or asking them to explain their actions I was repeatedly told "this conversation is over." The staff do not acknowledge someone standing at the desk to ask a question. They go about eating their meal in public. As unprofessional and low quality as it gets. I hope that this form a communication is actually listened to vs. disregarded.

My Dad was at Grand Village and he was neglected. He has a new medical condition where here he can't swallow and he was seen by the facility Nurse Practitioner and Physician at least twice weekly. They did not order any tests and did nothing to find the cause. He lost significant weight. I involved the Ombudsman and he was referred to an ENT who referred him to a fantastic neurologist, He had my Dad admitted on the same day to the hospital for failure to thrive, My Dad received two bags of IV fluids for dehydration last evening and he is already doing better. He's had a chest and throat x-ray and is having more tests. Grand village would have allowed my Dad to lose enough weight to where he would not have recovered. At 6'1", he is down to 145 pounds. A scan revealed that my Dad may have had two mini-strokes. He suffered for 4 weeks unnecessarily without any diagnosis nor treatment. How can a facility allow a human being to not eat or drink and continue to decline until the family ;intervenes and calls the Ombudsman. This is unacceptable and obvious negligence.

The facility was nice and the PT was good. However, I had no faith in the nursing manager or the follow up on anything. My mom was in there for rehab and she had hit here base line for medicare. We wanted to get her to an assisted living in the Twin Cities by us so we could visit her more and check on her care. We found a place that is really nice but weren't sure if they would take here as her care needs were a more than what they possibly could handle. Like she is on sliding scale insulin when they faxed the information to the new facility they left out a lot of things like she doesn't give the shots herself and at the new place the med techs can not give those shots. She is also on oxygen and I had given her new address to the nurse manager 5 days before my mom being discharged so they could get it set up for when she arrived. It wasn't there when we got there. Also, they said they would be discharged with physical therapy orders but they never did that either. Also, in their fax of her cares with the new place they left out a lot of stuff. The new place had her care level at a level 2 when it is really a level 6. She needs much more care than the new place can provide. Grand Village is poor on follow up. I wouldn't recomend them to anyone.
