Five Counties Nursing Home

Five Counties Nursing Home is a skilled-nursing care facility located in Lemmon, South Dakota, a town of approximately 1,200 people in Perkins County in the northwestern corner of the state. The facility was established as a nursing home addition in 1962, following the opening of Five Counties Hospital in 1951. Today, it is owned and operated by the Lemmon Area Medical Association, a nonprofit corporation established in 1988.
