About CenterWell Home Health

CenterWell Home Health in Brooklyn Park, Minnesota provides home health and hospice services for patients recovering from hospitalizations, surgeries, and serious health events. The team includes nurses, physical therapists, occupational therapists, and home health aides who visit patients at home.

Families consistently praise individual clinicians, nurses, therapists, and aides, for being attentive, skilled, and genuinely caring. One family wrote that their loved one "was treated with care, respect and dignity." Several reviews highlight therapists who spent significant time teaching exercises and setting recovery goals, and aides who brought warmth and humor to visits. However, communication and scheduling have emerged as real pain points: some families report staff arriving without advance notice or confirmation, missed follow-up appointments after discharge, and difficulty reaching the office to arrange care. One family described their experience as frustrating due to lack of coordination between office and field staff.

CenterWell Home Health holds a 4.6 out of 5 star rating across 52 Google reviews. The bulk of feedback is five-star, centered on skilled, compassionate individual caregivers; one-star reviews consistently point to administrative gaps in scheduling and responsiveness rather than quality of direct care.

My mother in law recently returned to her assisted living facility from a stay in rehab. This company has not shown up to 2 out of the 4 appointments. No phone call to reschedule. Thinking this is a medical scam. Bet they billed Medicare!

CenterWell cares about home care! My husband was visited at home by nurses, PT/OT, and an aide to help with personal care. He was returning from a long stay in the hospital and rehab. These clinicians were professional, personable, attentive and compassionate. They took vital signs, assessed and made recommendations, called the Dr. when needed, and set goals for the future.
The organization answered calls promptly and appropriately.
We were pleased ( an often impressed) by the care received.
Thank you CenterWell !

The Brooklyn Park team was just what I needed after back surgery. Professional, knowledgeable, kind and gentle describes their first rate quality of care. OT, PT, Social Work and Home Health Support all contributed their expertise toward my recovery. I am so grateful to all of them.

Does Medicare cover care from CenterWell Home Health?

Yes.
