My name is Ronald Williams you have a individual in there with my exact same name and birthday under your care. His info is causing me havoc on my med profile. My insurance believes I'm him and in this nursing home. When I'm not. Im in my home in Yantis texas now. His social is barely different from mine. Ill lose my Medicaid and insurance if someone doesn't fix the issue. UHC Coordinator of that facility had better get my info and his info corrected. It's causing medical misinformation.
