I’ve been at Boulevard Rehabilitation Center for over a month and a half. I had a total hip replacement at HSS in New York City and came down to rehab and learned to walk again while down here my gallbladder exploded and had to be removed do to the severe G.I. issues while here both of my ailments were treated in the most responsible manner, the staff here are very dedicated and knowledgeable, and they know what they are doing! My best advice to those who disagree with me should let them do their job and just cooperate it’s for your own best interest-the PT and OT therapist are professional and caring – the skilled nurses, ARE knowledgeable and attentive – administration staff are kind and always ready there to help – the maintenance crew keep this place pristine – and the kitchen prepare a wide variety of nutritious tasty cuisine every day– – thank you all for a job well done

I had a truly wonderful experience at Boulevard Rehabilitation Center, both as an inpatient and later through their outpatient physical therapy program. From the moment I arrived, the staff made me feel welcomed, supported, and genuinely cared for. Everyone I interacted with — nurses, therapists, and administrative staff — was kind, patient, and highly professional. The physical therapy team was exceptional. Whether I was inpatient or outpatient, they took the time to understand my needs, guided me through every exercise, and pushed me just the right amount to help me regain strength and mobility. Their knowledge, dedication, and encouragement made a tremendous difference in my recovery. The occupational therapy team was exceptional as well. They helped me rebuild confidence and independence in my daily activities, always explaining each step clearly and making sure I felt safe and capable. Their patience, creativity, and support were truly invaluable. The facility itself is clean, organized, and well‑run, which made every visit — inpatient or outpatient — smooth and stress‑free. I always felt respected, supported, and in good hands. I’m truly grateful for the excellent care I received and highly recommend Boulevard Rehabilitation Center — including their inpatient care, physical therapy, and occupational therapy — to anyone looking for a team that goes above and beyond.

My mom is in this place on S115 Caryn. And since the first day getting there she was so happy! What a difference from where she was. Just today they asked how everything was in her room. My mom stated, " There is some dust on things and maybe they can address it". We'll, My mom went to the lounge area and when she came back she was so elated. I am her son and she called me and said, " they cleaned this place so good. It's spick and span". The EVS workers and maintenance included deserve a kudos for the fastest response and thorough cleaning they did. They take care of her well also besides just this. Thanks to all.the nurses and staff! God Bless you all.

My grandmother was admitted to this facility for short-term rehabilitation following major surgery. Prior to surgery, she was completely independent and motivated to participate in physical and occupational therapy so she could return home safely. Unfortunately, her experience here was deeply distressing. During her stay, she called me multiple times in tears, stating that the way she was being treated made her feel hopeless and even expressed thoughts of wanting to harm herself with a plan to end her life. As a family member this is completely heartbreaking. No person should ever be made to feel that way, especially in a rehabilitation environment. I reached out directly to the administrator to report my concerns, including specific issues with a nurse whose behavior toward my grandmother felt bullying and unkind during an already incredibly vulnerable time. I did not receive a response, and nothing appeared to be done to address the situation. While I cannot speak to the long-term care side of this facility, I would absolutely not recommend it for anyone seeking short-term rehabilitation services. P.S. please don’t respond telling me to go to the ethics point website to file a report. I already reported this issue to the administrator and received no response. It is the responsibility of the facility to report this abuse/neglect issue as it’s a reportable offense.

After my father’s emergency surgery, he needed daily wound care and general assistance. After an initial stay at another facility, we were able to transfer him to The Boulevard, and the difference was immediately apparent. The staff was attentive and compassionate, the facility was clean and welcoming, the food was good, and having doctors on site provided real peace of mind. It was a huge relief to know my father was comfortable and well cared for. I highly recommend this rehabilitation facility to other families.
