Our family has been very pleased with the care and compassion shown to our dad at the Birkwood Assisted Living since he moved in last year. He often says ,"This isnt home," and of course nothing can truly replace home. But he also knows he is surrounded by caring staff, friends and a safe environment where he is well cared for. We especially appreciate all the many activities offered to residents. From games and social events to entertainment, exercise program, outings and holiday celebrations. There is always something available to help residents stay active and connected. Working in the healthcare myself, I tend to be very particular and and pay close attention to the quality of care , my dad receives. ALL the staff at the Birkwood is absolutely the best - from housekeeping, nursing/ caregiving, dietary, administration, and maintenance. Always attentive, professional and caring. They go above and beyond to make sure my dad is comfortable and supported. Knowing my dad is safe and cared for gives our family a peace of mind. We are grateful for everything Birkwood does for him and the other residents.

Our loved one was placed in the skilled nursing home at Birkwood Village but was moved into hospice care shortly afterwards. We can’t thank all the nurses and the entire staff for the care they gave her during her last days. Every single person we encountered was amazing to us. Highly recommend Birkwood Village, it was comforting to know they all cared so much for the patients and their families.

I am writing this review to share my serious concerns regarding the care and communication at Birkwood Village, where my grandmother recently was a resident. Despite repeated assurances from the care coordinator that I would be included in care meetings and kept in the loop, a major meeting was held on April 15 and May 29 without any notice to me. I was explicitly promised involvement and was deeply disappointed to be excluded. In a concerning turn, the care coordinator later claimed during my visit on May 14 that she “didn’t know I existed.” This is simply not true—she personally contacted me prior to my grandmother’s admittance to the facility to review her information and provide updates. This contradiction raises serious questions about accountability and transparency. Additionally, the care coordinator failed to follow through on her promises to help us find a suitable facility for my grandmother to transfer to. Despite multiple conversations, no real assistance or information was provided leaving us feeling abandoned in a critical time of need. Even more troubling, Birkwood Village failed to communicate with my grandmother’s designated medical power of attorney regarding her treatment or any changes to her care—something that should be standard protocol. To make matters worse, my grandmother was allowed to miss three dialysis treatments in a single week. When I visited and asked staff for an explanation, I was met with vague or dismissive responses. Finally, to add, she was transferred via ambulance to Blessing hospital two days after being discharged and admitted with critically low hemoglobin requiring blood transfusion, sepsis, and fluid overload. None of these issues started after discharge. While I know there are individual staff members who mean well, the systemic failures at Birkwood Village—especially around communication, follow-through, and basic health oversight—are unacceptable. Families deserve honesty, responsiveness, and a commitment to resident well-being. I sincerely hope this review leads to real improvements for all residents. — Carly Turcios

Birkwood is the perfect fit for our mom. Knowing she is being taken care of day and night. The activities they have for the residents is amazing. Thanks for being in Fort Madison

Three stars would be generous. The positive: they keep it adequately staffed and they are qualified, meals are fairly good. The negative: they hired someone who was convicted of elderly abuse and it took them three months to become compliant. Their sprinkler system is only in the hallways. And they had one fine for $36k+ . Nursing home failed to assist residents who need help with eating/drinking, grooming and personal and oral hygiene. Nursing home did not have record of quarterly fire drills for each shift under varying conditions.
