I highly recommend Avita Assisted Living for your loved one!! When my uncle's wife died unexpectedly, he needed a new home where he could receive some assistance with his care, as he has MS and some dementia. The entire staff, - leadership, nurses, chefs, dining attendants, caregivers, custodians, etc., - were genuinely caring, kind, attentive, and helpful. He loved living there and talking with other residents. He always raved about the food, that it was like fine dining. I ate with him a few times and it was high quality. He was able to bring his cat with him to live there and that was a comfort for him. Avita exceeded our expectations and we appreciated their all inclusive pricing, with no level of care fees. We love Avita and wish that they had communities in Texas!!

I am writing this to urge other families to be cautious when considering Avita for their loved ones. Our experience with the leadership team has been deeply concerning. Absentee Administration: The Administrator is consistently unavailable, remaining behind closed doors and making zero effort to engage with residents or build relationships with families. Dishonest Nursing Leadership: The two Directors of Nursing (DONs) have proven to be unreliable. We found them to be misleading and dishonest regarding care and protocols on multiple occasions. Retaliatory Environment: There is a "shut up or leave" culture here. When residents or family members advocate for better care by asking questions or filing complaints, the facility responds by discharging the resident rather than addressing the concerns. If you are looking for a facility that values open communication and treats residents with respect, I would suggest looking elsewhere.

My mom stayed at Avita Memory Care for 6+ years. There are good things but also tough things that I had to deal with especially considering the cost of her care. I agree with Susan McMullens comments as well. Keep your family doctor if you want them to consider the whole quality of life perspective because it’s a business. She had a pacemaker (another conversation) it would have been nice to have honest conversations about guidance/counseling for what is best for my beloved mom with dementia considering her end of life care wishes…you feel like your on an island that just wants to keep a body in the bed to pay the bills regardless of quality of life. Granted the right cardiologist is important too! Mom said, “I’m just a paycheck for someone now.” There was no consistent care…it’s a revolving door. No one has been directly there with my mom for the span of her stay. I get that it’s a hard environment. We are thankful for the love and care she did get. It allowed me to continue working. Hopefully Administration rethinks training and care. You fill out an information sheet for your loved ones family connections and history so staff can better relate to them…staff was not aware of her early life. Staff changes frequently, many times it’s fill in staff from an agency. They started with an activities director in 2019 … left in 2020. None til January 2025 and she is phenomenal! Think about that 4+years without activities for those with dementia. Hard to communicate with staff but There is now a message board that tells staff when residents have appointments after suggesting it. Meals are not served on a timely basis…many times I saw residents waiting at the table 30+ minutes before dinner was served…imagine having dementia and told to wait for food. Frustrating for staff as well. When I would go visit, many caregivers were on their phones not interacting with residents.Main dining … eek you feel like you’re intruding and not welcomed at all. Communication with staff is hard…can’t leave messages cause voicemail is full and they take forever to respond to emails. They don’t let you know when caregivers leave or new ones begin maybe rules but I’d like to know who’s with my mom…it would be nice if admin would send an email out to family to keep them in the loop. Nurses on staff will give you the cold shoulder, went to a potluck never acknowledged my mom or me just stood off to the side. They do let you know if she’s fallen or agitated but no communication of bright spots. Cpap machine did not get used regularly bc they rarely asked for more distilled water and would go in and see it wasn’t on, brushing teeth I wondered as well bc I never replenished toothpaste either…her teeth suffered even though I’d brush them when visiting. They have another side to memory care that focuses on diet, exercise, and therapies for those in the beginning stages but that costs twice as much. The week before she passed I was told by an agency nurse to contact a hospice care facility, staff said it wasn’t time…hindsight is 20/20 but they are the professionals! THIS makes me mad bc she was on hospice care just months before for so much less! I’m glad I did call Good Shepard bc they gave her a lavender bath (it had been at been over 2 weeks) and watched over her in her final days…yes days!..5 to be exact! I did not move my mom to another facility bc that would be super hard on her with her dementia and I had already moved her one other time. My mom passed away Sept. 10 we cleaned her things out Sept. 17… no condolences from nurses or admin. Nothing…just a paycheck except for Sara, the activities director who continues to be a godsend! She’s got the heart of an angel! So thankful for Sara, Alex in their care of my mom as I know they put up with a lot of her frustrations and confusion and others that I did not know as well. In reality, no one will care for your loved ones like you think they should be. Advocate for your loved ones! No place is perfect and this place has a lot to improve on..bc people matter…the whole person matters as they live and die!

Based on my personal experience, I cannot recommend this facility for elderly adults. In my time there, I observed inconsistent staff, including frequent use of agency employees who were not familiar with residents, and administration that did not seem to know or engage with those in their care. I also felt that families were misled about the level of care provided, and even basic considerations—like keeping visitor parking available—were not respected. From my experience, this facility is not equipped to handle increased care needs. My mother is no longer there, but I remain concerned for those who are. I share this so families can make informed decisions based on my personal observations

I have been blown away by the Clear Mind Center at Avita. The whole space is lovely, the staff is friendly, and the food is incredible. To be honest, this is somewhere I would totally want to live myself! I am impressed by how they handle the individual details of each case, how they respect our family's wishes, and how they update us all with photos and videos of our family member regularly so we are in the loop with how they are doing. They are also incredibly transparent which helps to build amazing trust. We are very satisfied with everything there and looking forward to further results from the center as well.
